【摘要】：目的:比較腎部分切除術(shù)(PN)和根治性腎切除術(shù)(RN)在T1b腎細(xì)胞癌(RCC)治療中的臨床療效,并為臨床操作的選擇提供循證醫(yī)學(xué)。方法:計(jì)算機(jī)檢索CKNI中文期刊全文數(shù)據(jù)庫(kù)、萬(wàn)方數(shù)據(jù)庫(kù)、維普數(shù)據(jù)庫(kù)、Medline、Pubmed、Cochrane圖書(shū)館。檢索時(shí)間:各數(shù)據(jù)庫(kù)自建庫(kù)至2016年12月01日。英文檢索詞:“partial nephrectomy”、“nephron sparing surgery”、“radical nephrectomy”、“renal cell carcinoma”、“T1b”、“pT1b”。中文檢索詞:“腎部分切除術(shù)”、“腎單位保留術(shù)”、“腎癌根治術(shù)”、“腎細(xì)胞癌”、“T1b”、“pT1b”。收集PN和RN在T1b期腎癌治療中的研究文獻(xiàn),并使用Meta分析比較生產(chǎn)率、局部復(fù)發(fā)及遠(yuǎn)處轉(zhuǎn)移。結(jié)果:共納入16篇研究文獻(xiàn),34357例腎癌患者,其中PN患者4014例,RN患者30343例,通過(guò)Mmeta分析評(píng)價(jià)兩種術(shù)式患者的5年總生存率(OS)(RR=1.05,95%CI為0.98~1.12,P=0.19)、10年總生存率(OS)(RR=1.07,95%CI為0.85~1.34,P=0.59)、5年無(wú)復(fù)發(fā)生存率(RFS)(RR=1.01,95%CI為0.97~1.06,P=0.54)、10年無(wú)復(fù)發(fā)生存率(RFS)(RR=1.06,95%CI為0.83~1.35,P=0.64)、5年癌癥特異性生存率(CSS)(RR=1.02,95%CI為0.97~1.08,P=0.36)、局部復(fù)發(fā)率(RR=2.19,95%CI為0.74~6.47,P=0.15)均無(wú)統(tǒng)計(jì)學(xué)差異;PN組10年癌癥特異性生存率(CSS)(RR=1.04,95%CI為1.03~1.05,P0.00001)、遠(yuǎn)處轉(zhuǎn)移發(fā)生率(RR=0.42,95%CI為0.21~0.86,P=0.02)均優(yōu)于RN組。結(jié)論:1、兩種術(shù)式患者的5年及10年OS、5年及10年RFS、5年CSS、局部復(fù)發(fā)率均無(wú)統(tǒng)計(jì)學(xué)差異;PN組10年CSS顯著優(yōu)于RN組、PN組腫瘤遠(yuǎn)處轉(zhuǎn)移發(fā)生率顯著低于RN組。2、與RN相比,在T1b期腎癌治療中PN避免了不必要腎單位的破壞,減少相關(guān)并發(fā)癥的發(fā)生,提高患者的生活質(zhì)量。因此,PN是值得推廣的一種手術(shù)方法。
[Abstract]:Objective: to compare the clinical efficacy of partial nephrectomy (PN) and radical nephrectomy (RN) in the treatment of T1b renal cell carcinoma (RCC) and to provide evidence-based medicine for the selection of clinical procedures. Methods: the full text database of CKNI Chinese journal, the Wanfang database and the Wiper database were searched by computer. Retrieval time: each database was built up to December 1, 2016. English keywords: "partial nephrectomy", "nephron sparing surgery", "radical nephrectomy", "renal cell carcinoma", "T1b", "pT1b". Chinese key words: partial nephrectomy, radical nephrectomy, renal cell carcinoma, T1b, pT1b. To collect the literature of PN and RN in the treatment of T1b renal cell carcinoma, and to compare the productivity, local recurrence and distant metastasis with Meta-analysis.
