【摘要】：針對(duì)CD19靶點(diǎn)的嵌合抗原受體(chimeric antigen receptor,CAR)T細(xì)胞在B細(xì)胞惡性血液系統(tǒng)腫瘤治療中取得的成功,證實(shí)了抗原特異性靶向免疫細(xì)胞對(duì)腫瘤治療的臨床價(jià)值。同時(shí)這一發(fā)現(xiàn)也激發(fā)了CAR-T細(xì)胞技術(shù)的拓展應(yīng)用性研究,多項(xiàng)CART細(xì)胞治療實(shí)體瘤的臨床前研究和臨床研究正在開展。然而實(shí)體腫瘤和血液腫瘤本身特質(zhì)上的差異決定了這種治療模式必須重新斟酌細(xì)胞治療本身的特點(diǎn)、作用規(guī)律等因素。本文討論了CAR-T細(xì)胞治療實(shí)體瘤相關(guān)的安全性管控措施、腫瘤靶點(diǎn)選擇、預(yù)處理及聯(lián)合治療,并通過相關(guān)的實(shí)踐經(jīng)驗(yàn)展望該治療方式的未來發(fā)展方向。
[Abstract]:The success of chimeric antigen receptor CART cells targeting CD19 targets in the treatment of malignant hematopoietic tumors of B cells has confirmed the clinical value of antigen-specific immunocytes in the treatment of tumors. At the same time, this discovery has stimulated the extensive application of CAR-T cell technology, and many preclinical and clinical studies of CART cell therapy for solid tumors are being carried out. However, the difference between solid tumor and blood tumor determines that this treatment mode must reconsider the characteristics and action rules of cell therapy itself. In this paper, the safety control measures, tumor target selection, preconditioning and combined therapy for solid tumor treated by CAR-T cells are discussed, and the future development of this treatment method is prospected through relevant practical experience.
