【摘要】：目的通過分析某醫(yī)院近三年肺癌住院患者費用構成及變化情況,探究影響其住院費用的關鍵因素,從而為費用控制策略的制定提供參考。方法從該院信息系統(tǒng)中抽取2014-2016年全部肺癌住院患者病案數(shù)據,應用趨勢檢驗、Spearman秩相關檢驗、多重線性逐步回歸等統(tǒng)計方法分析住院費用的變化趨勢和影響因素。結果 2014-2016年該院納入研究的3 364例肺癌患者以男性中老年為主,住院天數(shù)呈現(xiàn)正偏態(tài)分布,中位數(shù)為10天;三年間肺癌患者年平均住院日呈現(xiàn)逐年下降趨勢;放療人數(shù)呈現(xiàn)逐年下降趨勢;住院總費用和在院期間藥品費用具有高度相關性,且均呈逐年降低趨勢,年降幅分別為6.84%、10.88%和13.67%、20.81%;住院天數(shù)、是否放療、藥占比、是否手術及是否有合并癥為住院費用的影響因素。結論通過縮短平均住院日、控制藥占比、落實按病種管理等途徑可以有效降低肺癌患者住院費用,在保障醫(yī)療質量的同時減輕患者經濟負擔。
[Abstract]:Objective to analyze the cost composition and changes of lung cancer inpatients in a hospital in the past three years, and to explore the key factors that affect the cost of lung cancer, so as to provide a reference for the formulation of cost control strategy. Methods the medical records of all lung cancer inpatients from 2014-2016 were collected from the hospital information system. The trend test and Spearman rank correlation test, multiple linear stepwise regression and other statistical methods were used to analyze the change trend and influencing factors of hospitalization expenses. Results from 2014 to 2016, 3 364 lung cancer patients were mainly middle-aged and elderly men, with a positive distribution of hospitalization days (median of 10 days), and the average annual hospitalization days of lung cancer patients decreased year by year during the three years. The number of patients with radiotherapy showed a decreasing trend year by year, the total cost of hospitalization was highly correlated with the cost of drugs in hospital, and both showed a decreasing trend year by year, the annual decreases were 6.84% 10.88% and 13.67% 20.81%, respectively. Surgical treatment and complications were factors affecting hospital expenses. Conclusion by shortening the average hospitalization days, controlling the proportion of drugs and carrying out the management according to the type of disease, we can effectively reduce the hospitalization expenses of lung cancer patients, and reduce the economic burden of patients with lung cancer while ensuring the quality of medical treatment.
