【摘要】：目的：探討雷帕霉素靶蛋白(mammalian target of rapamycinm, mTOR)、血管內皮生長因子受體2(vascular epidemal growth factor receptor-2, VEGFR-2)、血小板衍生生長因子受體β(platelet-derived growth factor receptor beta, PDGFR-β)和CD117在軟組織肉瘤中表達的臨床意義及對預后的影響。方法：收集新疆醫(yī)科大學附屬腫瘤醫(yī)院骨與軟組織科2006年1月至2010年12月收治的94例軟組織肉瘤患者病理和臨床資料,應用組織芯片技術及PV-9000免疫組化方法檢測94例軟組織肉瘤中mTOR、VEGFR-2、PDGFR-β和CD117的表達,分析其與臨床病理特征和預后的關系。結果：1.94例軟組織肉瘤患者的中位PFS和OS分別為17個月(95%CI:10.639-23.361個月)和68.0個月(95%CI:61.806-74.194個月),3年生存率為71.0%,5年生存率為65.2%。2.mTOR、VEGFR-2、PDGFR-β和CD117在軟組織肉瘤中高表達率分別為：67.0%(63/94)、68.7%(64/94)、38.3%(36/94)、7.4%(7/94)。3.mTOR的表達與腫瘤病理類型,大小及臨床分期有關(P0.05),與性別、年齡、部位、深淺、組織學分級無關(P0.05)；VEGFR-2的表達與腫瘤病理類型、年齡有關(P0.05),與性別、腫瘤部位、大小、深淺、組織學分級、臨床分期無關(P0.05)；PDGFR-P的表達與腫瘤的深淺及組織學分級有關(P0.05)；CD117的表達與臨床病理特征間未見統(tǒng)計學意義的相關性。4.軟組織肉瘤的手術切緣、mTOR的表達與PFS/OS相關(P0.05)。結論：1.mTOR、VEGFR-2在軟組織肉瘤中高表達；2.mTOR和PDGFR-β高表達和陽性切緣是軟組織肉瘤預后不良指標；3.mTOR、VEGFR-2、 PDGFR-β在軟組織肉瘤中表達情況為軟組織肉瘤的靶向治療提供理論參考。免疫組化檢測CD117在軟組織肉瘤中的表達局限。
[Abstract]:Objective : To investigate the clinical significance and prognostic significance of rapamycin target protein ( mtor ) , vascular endothelial growth factor receptor - 2 ( VEGFR - 2 ) , platelet - derived growth factor receptor - 2 ( VEGFR - 2 ) , platelet - derived growth factor receptor beta ( PDGFR - 尾 ) and CD117 in soft tissue sarcoma .
The expression of VEGFR - 2 was related to the pathological type and age of tumor ( P0.05 ) . The expression of VEGFR - 2 was not related to sex , tumor site , size , depth , histological grade and clinical stage ( P0.05 ) .
The expression of PDGFR - P is related to the superficial and histological grade of tumor ( P0.05 ) .
There was no significant correlation between the expression of CD117 and clinicopathological features . 4 . The expression of mtor was correlated with PFS / OS in soft tissue sarcoma ( P0.05 ) . Conclusion : 1 . High expression of mtor and VEGFR - 2 in soft tissue sarcoma .
2 . High expression and positive margins of mtor and PDGFR - 尾 are poor prognostic indicators of soft tissue sarcoma ;
3 . The expression of mtor , VEGFR - 2 and PDGFR - 尾 in soft tissue sarcoma provides a theoretical reference for the targeted therapy of soft tissue sarcoma . The expression of CD117 in soft tissue sarcoma is limited by immunohistochemistry .
