【摘要】：目的探討腹腔鏡直腸癌低位前切除術(shù)后吻合口漏的影響因素。方法回顧性分析2008年7月~2016年12月我科腹腔鏡直腸癌低位前切除術(shù)568例的臨床資料。對(duì)可能影響吻合口漏發(fā)生的因素進(jìn)行單因素分析和多因素logistic回歸分析。結(jié)果出現(xiàn)吻合口漏22例(3.9%),其中A級(jí)10例,B級(jí)8例,C級(jí)4例。單因素分析結(jié)果顯示年齡、BMI、腫瘤距肛緣距離、術(shù)中是否保留左結(jié)腸動(dòng)脈與腹腔鏡直腸癌低位前切除術(shù)后吻合口漏有關(guān)(P0.05)。多因素分析結(jié)果顯示腫瘤距肛緣距離≤5 cm是腹腔鏡直腸癌低位前切除術(shù)后吻合口漏的獨(dú)立危險(xiǎn)因素(OR=0.054,95%CI 0.005~0.556,P=0.014),術(shù)中保留左結(jié)腸動(dòng)脈是吻合口漏的保護(hù)因素(OR=31.950,95%CI 2.804~364.056,P=0.005)。結(jié)論低位腫瘤是腹腔鏡直腸癌低位前切除術(shù)后吻合口漏的獨(dú)立危險(xiǎn)因素,而術(shù)中保留左結(jié)腸動(dòng)脈是吻合口漏的保護(hù)因素。
[Abstract]:Objective to investigate the influencing factors of anastomotic leakage after laparoscopic low anterior resection of rectal cancer.Methods the clinical data of 568 cases of low anterior resection of rectal cancer from July 2008 to December 2016 were retrospectively analyzed.Univariate analysis and multivariate logistic regression analysis were performed to analyze the factors that might affect the occurrence of anastomotic leakage.Results there were 22 cases of anastomotic leakage, including 10 cases of grade A, 8 cases of grade B and 4 cases of grade C.Univariate analysis showed that the age of BMI, the distance from the tumor to the anal margin, the preservation of the left colon artery during the operation and the leakage of anastomotic stoma after laparoscopic low anterior resection of rectal cancer were related to P0.05.Multivariate analysis showed that the distance from the tumor to the anal margin 鈮，
