【摘要】：目的觀察原發(fā)性肝癌(簡稱肝癌)射頻消融術(shù)后合并急性消化道出血的中西醫(yī)結(jié)合治療效果及對血管內(nèi)皮生長因子(VEGF)蛋白表達的影響。方法將43例符合要求的肝癌合并急性消化道出血患者隨機分為觀察組22例和對照組21例。觀察組以加減圣愈湯為主配合注射用血凝酶+埃索美拉唑鈉治療,對照組單純用注射用血凝酶+埃索美拉唑鈉治療。比較兩組止血療效、大便潛血轉(zhuǎn)陰時間、平均住院時間、不良反應率、VEGF蛋白表達水平。結(jié)果經(jīng)過觀察對比,觀察組止血有效率95.45%明顯高于對照組的76.19%(P0.05);觀察組總不良反應率明顯低于對照組(P0.05);觀察組潛血轉(zhuǎn)陰時間明顯低于對照組(P0.05);觀察組平均住院時間明顯低于對照組(P0.05)。結(jié)論以中藥圣愈湯加減配合西藥治療肝癌射頻消融術(shù)后合并消化道出血在提高止血療效、縮減大便潛血時間、降低藥物不良反應及縮短平均住院時間方面優(yōu)于單純西藥治療,同時可以提高機體免疫力,降低VEGF蛋白表達。
[Abstract]:Objective to observe the effect of integrated Chinese and western medicine on the expression of vascular endothelial growth factor (VEGF) protein after radiofrequency ablation of hepatocellular carcinoma (HCC) with acute gastrointestinal hemorrhage. Methods 43 patients with HCC complicated with acute gastrointestinal hemorrhage were randomly divided into two groups: the observation group (n = 22) and the control group (n = 21). The observation group was treated mainly with Jiajianshengyu decoction combined with esomeprazole sodium for injection, while the control group was only treated with esomeprazole sodium for injection. The hemostatic effect, the time of occult blood conversion, the average hospitalization time, the adverse reaction rate and the expression of VEGF protein were compared between the two groups. Results the effective rate of hemostasis was 95.45% in the observation group and 76.19% in the control group (P0.05), and the total adverse reaction rate in the observation group was significantly lower than that in the control group (P0.05). The time of occult blood turning negative in the observation group was significantly lower than that in the control group (P0.05), and the average hospitalization time in the observation group was significantly lower than that in the control group (P0.05). Conclusion the combination of Shengyu decoction and western medicine in the treatment of hepatoma combined with gastrointestinal bleeding after radiofrequency ablation is superior to that of western medicine alone in improving the hemostatic effect, reducing the time of occult blood in stool, reducing the adverse drug reaction and shortening the average hospital stay. At the same time, it can improve immunity and reduce the expression of VEGF protein.
