【摘要】：目的:對(duì)缺血性中風(fēng)急性期中西醫(yī)結(jié)合綜合治療方案在臨床中的應(yīng)用進(jìn)行隨機(jī)對(duì)照臨床試驗(yàn),觀察并評(píng)價(jià)其臨床療效。 方法:將所有入選病例隨機(jī)分為兩組,分別為中西醫(yī)結(jié)合綜合治療試驗(yàn)組和西醫(yī)綜合治療對(duì)照組,對(duì)兩組病例治療情況進(jìn)行觀測(cè)和記錄,用統(tǒng)計(jì)學(xué)方法處理相關(guān)數(shù)據(jù),進(jìn)行療效評(píng)價(jià)和科學(xué)總結(jié)。 結(jié)果:治療后兩組的臨床癥狀及體征均得到了改善,但兩組的改善特點(diǎn)存在差異。兩組療效、神經(jīng)功能缺損(NIHSS)評(píng)分、中醫(yī)病類診斷標(biāo)準(zhǔn)評(píng)分、中醫(yī)證候量化積分、總的生活能力狀態(tài)(ADL)評(píng)分及生存質(zhì)量(QLI)指數(shù)評(píng)分比較在治療后7、14、28天后的改變存在明顯差異。兩組患者治療后NIHSS評(píng)分均有明顯改善,在治療后14天時(shí)試驗(yàn)組優(yōu)于對(duì)照組(P0.05),28天時(shí)明顯優(yōu)于對(duì)照組(P0.01)。治療后28天時(shí)中醫(yī)病類診斷標(biāo)準(zhǔn)評(píng)分、中醫(yī)證候量化積分、ADL評(píng)分比較,試驗(yàn)組明顯優(yōu)于對(duì)照組(P0.01)。試驗(yàn)組在改善病人的生存質(zhì)量(QLI)指數(shù)方面明顯優(yōu)于對(duì)照組(P0.01)。治療后28天時(shí)試驗(yàn)組基本恢復(fù)病例占31.43%,顯著進(jìn)步者占65.71%,進(jìn)步者占2.86%,對(duì)照組基本恢復(fù)者占7.35%,顯著進(jìn)步者占22.06%,進(jìn)步者占66.18%,稍進(jìn)步者占4.41%,試驗(yàn)組臨床總療效優(yōu)于對(duì)照組(P0.05)。兩組TC、TG、LDL-C、Fg均有降低,試驗(yàn)組優(yōu)勢(shì)更加明顯(P0.01)。治療后對(duì)照組ALT明顯升高(P0.01),WBC明顯降低(P0.05)。 結(jié)論: 1.中西醫(yī)結(jié)合綜合治療方案合理應(yīng)用能夠降低缺血性中風(fēng)急性期患者死亡率,明顯促進(jìn)患者的神經(jīng)功能恢復(fù),改善患者的日常生活能力,提高患者遠(yuǎn)期生存質(zhì)量,降低致殘率。2.能夠較好地控制血脂異常,降低纖維蛋白原,改善凝血功能。
[Abstract]:Objective : To observe and evaluate the clinical curative effect of integrated traditional Chinese medicine and western medicine in the treatment of ischemic stroke . Methods : All selected cases were randomly divided into two groups , the combination of western medicine and western medicine combination therapy group and western medicine comprehensive treatment control group , the treatment of two groups were observed and recorded , the correlation data was treated by statistical method , and the curative effect evaluation and scientific summary were carried out . Results : Compared with the control group ( P0.01 ) , the clinical symptoms and signs of the two groups were significantly better than those in the control group ( P0.01 ) . Conclusion : 1 . Reasonable application of integrated traditional Chinese medicine and western medicine can reduce the mortality of patients in acute stage of ischemic stroke , obviously promote the recovery of nerve function of patients , improve the daily life ability of the patients , improve the long - term survival quality of the patients and reduce the disability rate .
