局部氧氣干預(yù)對(duì)高溫復(fù)合創(chuàng)傷大鼠創(chuàng)面愈合的影響

【摘要】:目的:通過(guò)建立高溫高濕環(huán)境下大鼠腹部創(chuàng)傷動(dòng)物模型,用局部氧氣干預(yù)創(chuàng)面的方法,探討這一特殊環(huán)境下創(chuàng)面組織的病理形態(tài)學(xué)變化以及局部氧氣干預(yù)對(duì)創(chuàng)面愈合的影響,為臨床傷口護(hù)理提供理論依據(jù)。方法:將24只創(chuàng)傷大鼠隨機(jī)分為常溫對(duì)照組(NEC)、常溫氧療組(NEO)、高溫對(duì)照組(HHE)、高溫氧療組(HHO)4組,每組6只。在相應(yīng)干預(yù)后的不同時(shí)間對(duì)各組動(dòng)物的平均動(dòng)脈壓(MAP)、創(chuàng)面愈合時(shí)間、細(xì)菌學(xué)定量、病理形態(tài)學(xué)進(jìn)行觀察和檢測(cè)。結(jié)果:HHE組較NEC組創(chuàng)面腫脹程度嚴(yán)重,創(chuàng)面愈合時(shí)間長(zhǎng),HHO組較HHE組創(chuàng)面腫脹程度輕,創(chuàng)面愈合時(shí)間短。光鏡和電鏡觀察到HHE組創(chuàng)面肌肉纖維的病理變化均較NEC組顯著,且損傷呈進(jìn)行性加重,HHO組肌肉組織損傷程度較HHE組輕。細(xì)菌學(xué)定量檢測(cè)HHE組創(chuàng)面細(xì)菌數(shù)在同一時(shí)相比NEC組高,且隨時(shí)間的延長(zhǎng)呈顯著增長(zhǎng)趨勢(shì),HHO組創(chuàng)面細(xì)菌數(shù)量與HHE組比較差異有顯著性意義(P0.05)。結(jié)論:高溫高濕環(huán)境下創(chuàng)面組織病理?yè)p傷嚴(yán)重,細(xì)菌繁殖快,感染時(shí)間提前且嚴(yán)重,創(chuàng)面愈合速度慢;局部氧氣干預(yù)可抑制創(chuàng)面組織細(xì)菌繁殖,對(duì)大鼠腹部刀割創(chuàng)面有明顯的促修復(fù)作用。
[Abstract]:Objective: to investigate the pathomorphological changes of wound tissue and the effect of local oxygen intervention on wound healing by establishing an animal model of abdominal trauma in rats under high temperature and high humidity. To provide theoretical basis for clinical wound nursing. Methods: 24 traumatized rats were randomly divided into normal temperature control group (NEC), (NEO), hyperthermia control group, (HHE), hyperthermia oxygen therapy group (HHO) 4), 6 rats in each group. The wound healing time, bacteriological quantity and pathomorphology of (MAP), wound were observed and detected at different time after intervention. Results: the degree of wound swelling in HHE group was more serious than that in NEC group, and the wound healing time was longer. In HHO group, the degree of wound swelling was lighter and the healing time was shorter than that in HHE group. The pathological changes of muscle fibers in HHE group were significantly higher than those in NEC group, and the injury was progressive. The degree of muscle tissue injury in HHO group was lighter than that in HHE group. The number of bacteria in wound of HHE group was higher than that of NEC group at the same time, and the number of bacteria in HHO group was significantly higher than that in HHE group (P0.05). Conclusion: under high temperature and high humidity environment, the wound tissue has serious pathological damage, the bacteria propagate fast, the infection time is early and serious, and the wound healing speed is slow. Local oxygen intervention could inhibit the propagation of bacteria in wound tissue and could promote the repair of abdominal incision wound in rats.
