不同復溫方式對犬腹部開放傷合并海水浸泡救治時血流動力學的影響

【摘要】:目的 探討不同復溫方式對犬腹部開放傷合并海水浸泡救治時血流動力學的影響。方法 成年雜種犬 30只制作成腹部開放傷動物模型 ,傷后均浸泡人工海水中 ,2h后打撈出水 ,隨機分為 3組。自然復溫組 (n=10 ) :將動物移至溫暖環(huán)境 ,靠機體自身產(chǎn)熱自發(fā)復溫 ;體表復溫組 (n=10 ) :用電熱毯體表加溫 ;體中心復溫組 (n=10 ) :1/3張鹽水 4 2℃水浴 2 0min后 ,腹腔用 1/2張鹽水持續(xù)灌洗 2h。觀察不同復溫方式對犬腹部開放傷合并海水浸泡救治時體溫、心率、平均動脈壓、中心靜脈壓和心輸出量的影響。結(jié)果 自然復溫組復溫效果不滿意 ,救治 6h后體溫 34 8℃ ,未恢復正常體溫 ,血壓提升困難、心動過速、心輸出量降低。體表復溫組復溫效果優(yōu)于前組 ,6h后體溫 36 9℃ ,接近致傷前體溫 ,但血壓、心率和心輸出量也出現(xiàn)同前組類似的表現(xiàn) ,只是程度較輕。體中心復溫組復溫效果滿意 ,救治后 4h體溫、平均動脈壓、中心靜脈壓和心輸出量基本恢復正常。結(jié)論 腹部開放傷海水浸泡后體溫過低的救治時最好采用體中心復溫方式。
