維和衛(wèi)勤保障中的特殊醫(yī)德問(wèn)題研究

【摘要】: 我國(guó)古代有“上醫(yī)醫(yī)國(guó)、中醫(yī)醫(yī)人、下醫(yī)醫(yī)病。有道醫(yī)靈魂,無(wú)道不成醫(yī)”之說(shuō),即上醫(yī)能夠醫(yī)治國(guó)家,中醫(yī)能夠醫(yī)治人的思想靈魂,下醫(yī)則只能醫(yī)治人的肉體。對(duì)于一名醫(yī)生來(lái)說(shuō),具備高尚的醫(yī)德,遠(yuǎn)比僅僅憑借藥物來(lái)去邪治病更為重要。 隨著現(xiàn)代科學(xué)技術(shù)的迅猛發(fā)展,醫(yī)學(xué)模式的轉(zhuǎn)變,社會(huì)環(huán)境的變化亦引起了醫(yī)德基本范疇的不斷拓展和深化。醫(yī)德在不同的領(lǐng)域、不同的時(shí)期表現(xiàn)出不同的問(wèn)題,即就需要特殊的醫(yī)德原則和規(guī)范。維和等境外領(lǐng)域開(kāi)展醫(yī)療救助就有其特殊的醫(yī)德要求。 衛(wèi)勤部隊(duì)是我軍重要的組成部分,在信息化戰(zhàn)爭(zhēng)中的地位、作用顯得更加突出。在新的歷史條件下,研究我軍在維和等境外環(huán)境衛(wèi)勤保障中的特殊醫(yī)德問(wèn)題,必須堅(jiān)持歷史與邏輯、繼承與創(chuàng)新、理論與實(shí)踐相統(tǒng)一的方法論原則,堅(jiān)持系統(tǒng)方法、開(kāi)放方法、實(shí)驗(yàn)方法、實(shí)踐方法、綜合方法等具體方法。 本研究從普遍性與特殊性相結(jié)合的角度,對(duì)聯(lián)合國(guó)授權(quán)下的國(guó)際性維和行動(dòng)及我國(guó)參與的維和任務(wù)進(jìn)行了分析,指出在當(dāng)今世界局勢(shì)條件下,參與維和任務(wù)與衛(wèi)勤保障所應(yīng)遵守的醫(yī)德原則:聯(lián)合國(guó)受命原則;受援國(guó)同意原則;出兵國(guó)中立原則;和平與穩(wěn)定原則;負(fù)責(zé)任原則和增進(jìn)友誼原則。 借助《日內(nèi)瓦公約》的相關(guān)內(nèi)容,對(duì)人道主義精神與衛(wèi)勤保障的醫(yī)德要求進(jìn)行了初步論述,總結(jié)了我軍在以往戰(zhàn)爭(zhēng)中發(fā)揚(yáng)人道主義精神所遵循的規(guī)范。其主要表現(xiàn)在:在政治上尊重人格;生活上和精神上給予其特殊的待遇;實(shí)行“救死扶傷”,慎重處理陣亡人員;尊重宗教信仰;免費(fèi)郵寄信函等物;積極組織遣返等。 通過(guò)調(diào)查研究發(fā)現(xiàn),我軍在執(zhí)行聯(lián)合國(guó)維和任務(wù)時(shí),所處的衛(wèi)勤保障環(huán)境與國(guó)內(nèi)相比明顯不同,醫(yī)療條件不同,面對(duì)的人際關(guān)系與國(guó)內(nèi)有著根本差異以及遇到的傷病譜系也可能與國(guó)內(nèi)有很多不同。衛(wèi)勤保障人員面對(duì)著很多未知的情況,可能會(huì)遇到從未見(jiàn)過(guò)的問(wèn)題。因此,在維和條件下,我軍醫(yī)務(wù)工作者必須堅(jiān)守以下特殊醫(yī)德理念:一是保障戰(zhàn)斗力首位;二是醫(yī)生要與士兵同在;三是衛(wèi)勤先行;四是尊重公序良俗;五是搶救順序先搶后救、先重后輕、先急后緩、先近后遠(yuǎn);六是積極后送施救;七是必要時(shí)采取緊急避險(xiǎn);八是要關(guān)愛(ài)亡者;九是在戰(zhàn)后積極消除戰(zhàn)爭(zhēng)后遺癥;十是對(duì)相關(guān)信息保密,不予告知。 在新形勢(shì)下,針對(duì)軍隊(duì)特殊醫(yī)德問(wèn)題研究的比較少,特殊醫(yī)德問(wèn)題教育的比較少等軍隊(duì)醫(yī)德建設(shè)現(xiàn)狀,本研究試圖從醫(yī)學(xué)倫理學(xué)、軍事社會(huì)學(xué)、戰(zhàn)爭(zhēng)法學(xué)等角度,有針對(duì)性的探尋特殊醫(yī)德素質(zhì)的培養(yǎng)方法。提出了:一是開(kāi)拓視野,加強(qiáng)特殊環(huán)境下衛(wèi)勤保障中的醫(yī)德研究;二是深化教育,熟悉適應(yīng)特殊環(huán)境下衛(wèi)勤保障中的醫(yī)德要求;三是模擬實(shí)戰(zhàn),在實(shí)踐中強(qiáng)化特殊環(huán)境下軍醫(yī)的職業(yè)道德素質(zhì);四是比較吸收,借鑒外軍在特殊環(huán)境下處理醫(yī)德問(wèn)題的經(jīng)驗(yàn);五是建立制度,完善特殊環(huán)境下軍隊(duì)醫(yī)德職業(yè)的特殊規(guī)范等一套科學(xué)性和時(shí)效性較強(qiáng)的特殊醫(yī)德規(guī)范教育對(duì)策。
[Abstract]:In ancient China , there were " upper medical doctors , traditional Chinese medicine doctors , and lower medical doctors " . It is said that the upper medicine can cure the country , the traditional Chinese medicine can cure the soul of the human mind , and the lower medicine can only cure the human flesh . For a doctor , it is more important than merely relying on drugs to treat the disease .


With the rapid development of modern science and technology , the transformation of medical model and the change of social environment also cause the expanding and deepening of the basic category of medical ethics .


It is an important part of our army to study the status and function of our army in the information war . Under the new historical conditions , we must adhere to the methodology principles of the unification of history and logic , inheritance and innovation , theory and practice , persist in systematic methods , open methods , experimental methods , practical methods , comprehensive methods and so on .


From the perspective of universality and particularity , this study analyses the international peacekeeping operation under the authority of the United Nations and the peacekeeping missions involved in our country , and points out that in the current world situation , the principles of medical ethics that should be observed in the field of peacekeeping missions and security : the principle of the United Nations , the principle of consent of recipient countries , the principle of neutrality of troop - contributing countries , the principle of peace and stability , the principle of responsible principle and the principle of friendship and friendship .


With the help of the relevant contents of the Geneva Conventions , the medical and moral requirements of humanitarian spirit and security are discussed , and the norms that our army has followed in the past wars are summarized . The main manifestations are : respect for personality in politics ; life and spirit ; respect for religious belief ; free postal correspondence ; and active organization of repatriation .


According to the investigation , we found that in the implementation of the United Nations peacekeeping mission , the security environment of our army is obviously different from that in the country , the medical conditions are different , the relationship with the country has the fundamental difference and the injury spectrum is likely to be different from the country . Therefore , in the situation of peacekeeping , the medical personnel of our army must adhere to the following special medical ethics idea : First , to guard the battle force first ; secondly , to respect the public order ; thirdly , to take emergency contraception after the rescue sequence ;


In the new situation , according to the relatively few research on the special medical ethics problem of the army and the comparatively small education of special medical ethics , this paper tries to find out the training methods of special medical ethics quality from the angles of medical ethics , military sociology , war law and so on .
