解放軍小湯山醫(yī)院患者及一線醫(yī)護人員心理干預模式研究

【摘要】:目的 1、對解放軍小湯山醫(yī)院SARS患者實施整體心理危機干預措施并評估療效和影響因素,探索對危機人群進行臨床心理干預的有效模式。 2、對解放軍小湯山醫(yī)院一線醫(yī)護人員進行心理干預并評估療效,探索對急性應激事件下的人員進行臨床心理干預的有效模式。 方法 1、680名SARS患者(其中男性379名,女性301名),對483名干預組患者進行綜合心理干預,包括電話心理咨詢(主動和被動)、文字材料、自我調(diào)適以及面對面危機干預(主要由臨床醫(yī)務人員和心理醫(yī)生實施)。采用SCL-90對干預組和對照組進行入院時和出院前的心理健康狀況評估,并采用自編的“心理干預效果評定表”對各種干預方法的干預效果進行評定。 2、輪休期SARS一線醫(yī)護人員265人,對實驗組131名一線醫(yī)護人員進行心理行為訓練,對照組134人不進行心理行為訓練,用SCL-90評估干預效果。 結(jié)果 1、實驗組和對照組患者入院時SCL-90各因子分值均明顯高于地方常模的分值,差異有顯著性;入院時干預組和對照組的SCL-90各因子均分沒有明顯差異,而出院時干預組和對照組患者SCL-90各因子均分相比,在多個因子上干預組均分均顯著低于對照組(t=2.1~5.7,P0.01~0.05),兩組人員的各因子得分仍然顯著高于地方常模。各種干預方式中患者主動電話咨詢的有效率最高(95.7%),文字材料的有效率最低(19.5%),經(jīng)過培訓后的臨床主管醫(yī)生與心理醫(yī)生進行心理干預的干預有效率都很高(80%),兩者之間的有效率比較無顯著性差異;主動求助、被動求助以及文字干預對文化程度較低的
[Abstract]:Purpose. 1. To carry out the whole psychological crisis intervention to SARS patients in Xiaotangshan Hospital of PLA, to evaluate the curative effect and the influencing factors, and to explore the effective mode of clinical psychological intervention to the people in crisis. 2. Psychological intervention and evaluation of the curative effect of first-line medical staff in Xiaotangshan Hospital of PLA were carried out to explore the effective mode of clinical psychological intervention for the personnel under acute stress events. Method. 1,680 patients with SARS (1,680 males and 301females) underwent comprehensive psychological intervention, including telephone counseling (active and passive, text materials). Self adjustment and face to face crisis intervention (mainly carried out by clinical medical staff and psychiatrist. Mental health status of intervention group and control group were evaluated by SCL-90 at admission and before discharge. The psychological intervention effect evaluation table was used to evaluate the intervention effect of various intervention methods. 2. During the rest period, 265 first-line medical staff of SARS were trained in psychological behavior in the experimental group, while those in the control group were not trained in psychological behavior. SCL-90 was used to evaluate the effect of intervention. Results. 1. The scores of SCL-90 factors in the experimental group and the control group were significantly higher than those of the local norm at admission, and there was no significant difference between the intervention group and the control group at admission. When discharged from hospital, the mean scores of SCL-90 factors in the intervention group and the control group were compared. In many factors, the average score of intervention group was significantly lower than that of control group. The scores of each factor of the two groups were still significantly higher than that of the local norm. The highest effective rate of patient active telephone consultation was 95.7% and the effective text material was effective. The lowest rate was 19.5%. The effective rate of psychological intervention between the clinical doctor and the psychologist after training was very high, and there was no significant difference between the two.
