【摘要】：目的探討腎嗜酸細胞腺瘤與腎細胞癌的CT鑒別診斷要點。方法回顧性分析經手術病理證實的9例腎嗜酸細胞腺瘤與33例腎細胞癌的CT資料。結果 9例腎嗜酸細胞腺瘤均為單發(fā),平掃9例皆密度均勻或主體密度均勻,其中8例呈等或低密度,邊緣不清,1例呈稍高密度,5例中央可見低密度瘢痕影;增強掃描5例表現為一過性強化,中央可見星狀或小片狀無明顯強化區(qū);4例表現為快進緩出型強化,皮髓質期不均勻強化,實質期、排泄期強化趨于均勻。腎細胞癌33例中,平掃22例呈低、等或混雜密度,增強掃描27例表現為不均勻一過性強化;14例可見靜脈瘤栓及其他臟器轉移等征象。結論中央星狀瘢痕和延遲期均勻強化有助于腎嗜酸細胞腺瘤與腎細胞癌的鑒別診斷。
[Abstract]:Objective to study the CT differential diagnosis between renal eosinophil adenoma and renal cell carcinoma. Methods CT findings of 9 cases of renal eosinophils adenoma and 33 cases of renal cell carcinoma confirmed by surgery and pathology were analyzed retrospectively. Results 9 cases of renal eosinophils adenoma were single, 9 cases of plain scan were homogeneous density or homogeneity of main body density, 8 cases showed equal or low density, 1 case showed slightly high density and 5 cases had low density scar in the center. Enhanced scan showed transient enhancement in 5 cases, and in the central stellate or small flake without obvious enhancement in 4 cases, rapid in and slow out enhancement, uneven enhancement in medulla, uniform enhancement in parenchymal and excretory phase in 4 cases. In 33 cases of renal cell carcinoma, 22 cases showed low, equal or mixed density on plain scan, and 27 cases showed heterogeneous transient enhancement in 27 cases with venous thrombus and other viscera metastasis. Conclusion uniform enhancement of central stellate scar and delayed phase is helpful for differential diagnosis of renal eosinophil adenoma and renal cell carcinoma.
