肺血管CT成像技術(shù)的優(yōu)化及其初步臨床應(yīng)用研究

【摘要】:第一部分肺血管CT成像技術(shù)的優(yōu)化 目的:通過四個(gè)階段分別對(duì)不同對(duì)比劑注射路徑、不同掃描方向、不同對(duì)比劑用量及不同掃描延遲時(shí)間對(duì)肺動(dòng)脈、肺靜脈和主動(dòng)脈強(qiáng)化程度的影響,最終獲得優(yōu)化的肺血管CT成像技術(shù)。 資料和方法:收集行CT胸部增強(qiáng)檢查的患者共270例,隨機(jī)分為9組,每組30例。研究分為四個(gè)階段,第一階段對(duì)比劑經(jīng)左側(cè)或右側(cè)肘靜脈注射;第二階段掃描方向?yàn)轭^-足側(cè)或足-頭側(cè);第三階段對(duì)比劑用量為70ml或90ml;第四階段掃描延遲時(shí)間為16s、18s或20s。使用64排螺旋CT機(jī),管電壓120kV,管電流200mAs,準(zhǔn)直寬度0.625mm×64,SFOV50cm,DFOV36cm,層厚5mm。將獲得的圖像進(jìn)行0.625層厚重建傳至后處理工作站,測(cè)量肺動(dòng)脈、肺靜脈及降主動(dòng)脈的強(qiáng)化CT值,并對(duì)上腔靜脈偽影以4分法做出評(píng)分,計(jì)算肺動(dòng)脈和降主動(dòng)脈強(qiáng)化CT值≥250Hu的百分率;結(jié)果比較每階段組間肺動(dòng)脈、靜脈、降主動(dòng)脈的CT值、上腔靜脈偽影、肺動(dòng)脈和降主動(dòng)脈CT值≥250Hu的百分率。 結(jié)果:肺動(dòng)脈和降主動(dòng)脈CT值≥250Hu的百分率各階段組間無差異。右側(cè)肘靜脈注射組肺動(dòng)脈、肺靜脈和降主動(dòng)脈的CT值高于左側(cè)組,兩組整體無差異;足-頭側(cè)掃描組上腔靜脈偽影和肺靜脈CT值低于頭-足側(cè)組,差異有統(tǒng)計(jì)學(xué)意義。90ml對(duì)比劑用量組肺動(dòng)脈和降主動(dòng)脈CT值高于70ml組,差異有統(tǒng)計(jì)學(xué)意義;肺動(dòng)脈強(qiáng)化CT值16s組≈18s組20s組,肺靜脈強(qiáng)化CT值16s組18s≈20s組,差異有統(tǒng)計(jì)學(xué)意義。降主動(dòng)脈強(qiáng)化無差異。上腔靜脈偽影延遲16s組18s組20s組。 結(jié)論:右側(cè)肘靜脈注射對(duì)比劑、足-頭側(cè)掃描、對(duì)比劑用量90ml300mg I/ml,延遲20s為推薦的肺血管CT成像優(yōu)化技術(shù)。第二部分肺血管CT成像的初步臨床應(yīng)用 目的:分析肺部病變與血管的關(guān)系,探討肺血管CT成像在肺部疾病的診斷及鑒別診斷中的應(yīng)用價(jià)值。 資料和方法:回顧性分析肺動(dòng)脈和降主動(dòng)脈強(qiáng)化值≥250Hu的患者圖像資料共58例,其中肺癌32例,肺栓塞4例,肺炎性假瘤1例,肺膿腫1例,肺結(jié)核6例,肺硬化性血管瘤2例,肺隔離癥2例,肺海綿狀血管瘤1例,肺炎9例。將圖像進(jìn)行血管容積再現(xiàn)(VR)、最大密度投影(MIP)、多平面重組(MPR)、曲面重組(CPR)、最小密度投影(Min IP)等后處理重建,評(píng)價(jià)病變與血管的關(guān)系。 結(jié)果:重建圖像上,肺癌肺動(dòng)脈、肺靜脈可表現(xiàn)為截?cái)、受侵或受壓;支氣管?dòng)脈可增粗。肺栓塞肺動(dòng)脈內(nèi)可見充盈缺損;肺硬化性血管瘤血管表現(xiàn)為“貼邊血管征”;肺隔離癥可見供血?jiǎng)用}和引流靜脈;肺炎性病變血管可增粗、扭曲或正常。 結(jié)論:肺血管CT成像在肺部疾病的診斷和鑒別診斷中有重要的價(jià)值。
[Abstract]:The optimization of CT imaging in the first part of the lung

Objective : To study the influence of different contrast agent injection path , different scanning direction , different contrast agent dosage and different scanning delay time on the degree of pulmonary artery , pulmonary vein and aorta enhancement through four stages , and finally obtain optimized pulmonary vascular CT imaging technique .

Materials and Methods : A total of 270 patients with CT chest enhancement examination were randomly divided into 9 groups , 30 patients in each group . The study was divided into four stages , the first phase contrast agent was injected intravenously via left or right elbow ;
the second stage scanning direction is the head - foot side or the foot - head side ;
the dosage of the third phase contrast agent is 70ml or 90ml ;
Using 64 - slice spiral CT machine , tube voltage 120kV , tube current 200mAs , collimation width 0.625mm 脳 64 , SFOV50 cm , DFOV36cm , layer thickness 5mm . The obtained image was reconstructed to post - treatment workstation to measure the enhanced CT value of pulmonary artery , pulmonary vein and descending aorta .
