【摘要】：目的探討鉬銠雙靶X線攝影乳腺腫塊邊緣征象及周圍結(jié)構(gòu)對(duì)良惡性病變的診斷價(jià)值。方法搜集2014—2015年度經(jīng)手術(shù)及病理證實(shí)的220例乳腺腫塊患者的臨床資料,回顧性分析鉬銠雙靶X線攝影乳腺腫塊邊緣征象及周圍結(jié)構(gòu)情況,分析X線征象在良惡性病變中的檢出率差異及X線征象的良惡性診斷結(jié)果與病理診斷間的一致性。結(jié)果邊緣清晰、周圍結(jié)構(gòu)正常在X線診斷良性患者的檢出率高于X線診斷惡性患者(P0.01);邊緣浸潤(rùn)、星芒狀及周圍結(jié)構(gòu)扭曲在X線診斷惡性患者的檢出率高于X線診斷良性患者(P0.01)。邊緣浸潤(rùn)、星芒狀及周圍結(jié)構(gòu)扭曲在惡性病變的檢出率高于良性病變(P0.01)。乳腺腫塊邊緣征象及周圍結(jié)構(gòu)在良惡性病變?cè)\斷中的敏感度、特異度、準(zhǔn)確性、陽(yáng)性預(yù)測(cè)值及陰性預(yù)測(cè)值分別為86.4%、91.8%、89.1%、91.3%、87.1%。X線征象的良惡性診斷結(jié)果與病理診斷具有較好的一致性(Kappa=0.782,P0.001)。結(jié)論鉬銠雙靶X線攝影乳腺腫塊邊緣征象及周圍結(jié)構(gòu)對(duì)良惡性病變的鑒別中具有重要價(jià)值,該檢查簡(jiǎn)單、易操作,具有重要的臨床意義。
[Abstract]:Objective to investigate the diagnostic value of margin signs and peripheral structure of mammography with molybdenum-rhodium double-target mammography for benign and malignant lesions. Methods the clinical data of 220 patients with breast masses confirmed by surgery and pathology in 2014-2015 were collected and the marginal features and surrounding structure of mammography with molybdenum rhodium double target radiography were analyzed retrospectively. To analyze the difference of the detection rate of X-ray signs in benign and malignant lesions and the consistency between the results of benign and malignant diagnosis of X-ray signs and pathological diagnosis. Results the positive rate of normal peripheral structure in benign patients was higher than that in malignant patients diagnosed by X ray, and the detection rate of margin infiltration, star and surrounding structure distortion in X-ray diagnosis of malignant patients was higher than that in X-ray diagnosis of benign patients (P 0.01). The detection rate of marginal invasion, stellate and surrounding structure distortion in malignant lesions was higher than that in benign lesions (P 0.01). The sensitivity, specificity, accuracy, positive predictive value and negative predictive value of the marginal signs and surrounding structures of breast masses in the diagnosis of benign and malignant lesions were 86.4and 91.81.The results of the benign and malignant diagnosis of the X-ray signs were 91.3 and 87.1respectively, which were in good agreement with the pathological diagnosis. Conclusion the marginal signs and surrounding structures of mammography with molybdenum rhodium double target radiography are of great value in the differential diagnosis of benign and malignant lesions. This examination is simple, easy to operate and has important clinical significance.
