【摘要】：目的對(duì)比容積CT數(shù)字減影血管造影(VCTDSA)和三維數(shù)字減影血管造影(3D-DSA)在顱內(nèi)動(dòng)脈瘤診斷中的價(jià)值。方法疑似顱內(nèi)動(dòng)脈瘤患者74例,男31例,女43例,平均72.3歲。均行VCTDSA和3D-DSA檢查,兩位評(píng)價(jià)者分析此兩項(xiàng)檢查的影像資料,記錄動(dòng)脈瘤檢出的個(gè)數(shù)、部位及大小,并評(píng)價(jià)VCTDSA的診斷效能(包括敏感性、特異性、陽(yáng)性預(yù)測(cè)值和陰性預(yù)測(cè)值)及兩種技術(shù)診斷動(dòng)脈瘤的一致性。結(jié)果 3D-DSA檢出動(dòng)脈瘤94個(gè),VCTDSA檢出93個(gè)(評(píng)價(jià)者A檢出93個(gè),評(píng)價(jià)者B 92個(gè));評(píng)價(jià)者A檢出全部動(dòng)脈瘤以及檢出3 mm動(dòng)脈瘤的敏感性和陰性預(yù)測(cè)值較評(píng)價(jià)者B高,而兩者檢出≥3 mm動(dòng)脈瘤的敏感性和特異性均為100%;在檢出動(dòng)脈瘤個(gè)數(shù)、部位和大小三方面,3D-DSA和VCTDSA有很好的評(píng)價(jià)者間的一致性。結(jié)論 VCTDSA的動(dòng)脈瘤檢出效能與3D-DSA相當(dāng),可以作為顱內(nèi)動(dòng)脈瘤影像診斷的準(zhǔn)確方法。
[Abstract]:Objective to compare the value of volume CT digital subtraction angiography (VCTDSA) and three-dimensional digital subtraction angiography (3D-DSA) in the diagnosis of intracranial aneurysms. Methods 74 cases of suspected intracranial aneurysms, 31 males and 43 women, averaging 72.3 years old. All of them were examined by VCTDSA and 3D-DSA, and two of the two patients were analyzed the two examination images and recorded the aneurysm examination. The number, location, size and diagnostic efficacy of VCTDSA (including sensitivity, specificity, positive predictive value and negative predictive value) and the consistency of two kinds of aneurysms were evaluated. Results 3D-DSA detected 94 aneurysms and 93 VCTDSA (93 of the evaluators were detected by A and 92 of the evaluators); the evaluator A detected all the aneurysms and detected 3 of the aneurysms. The sensitivity and negative predictive value of MM aneurysm were higher than that of the evaluator B, and the sensitivity and specificity of both of them to 3 mm aneurysms were 100%. In detecting the number of aneurysms, location and size three, 3D-DSA and VCTDSA had a good consistency between the evaluators. Conclusion the detection efficiency of the aneurysm of VCTDSA is equivalent to that of 3D-DSA and can be used as a intracranial aneurysm. An accurate method of imaging diagnosis of aneurysm.
