【摘要】：目的探討結(jié)合偽彩成像提高胸部CT平掃乳腺癌檢出率的價值。方法搜集經(jīng)病理證實(shí)的乳腺癌患者30例(共60只乳腺),由兩名醫(yī)師分別獨(dú)立閱讀胸部CT平掃縱隔窗圖像判斷是否存在乳腺癌病灶,2周后應(yīng)用胸部平掃CT圖像結(jié)合偽彩成像技術(shù)閱片判斷是否存在乳腺癌病灶。采用Kappa方法計算兩名醫(yī)師的一致性,采用卡方檢驗(yàn)驗(yàn)證結(jié)合偽彩成像技術(shù)可否提高胸部CT平掃乳腺癌檢出率。結(jié)果纖維腺體松散分布的乳腺(Ⅰ類,13例)中,乳腺癌灶的CT值平均值高于正常腺體的CT值平均值[(21.0±13.0)HU vs(-28.1±32.8)HU](P0.001),纖維腺體致密分布的乳腺(Ⅱ類,17例)中亦如此[(34.3±12.5)HU vs(1.9±21.7)HU](P0.001)。在Ⅱ類乳腺中,兩名醫(yī)師在CT平掃圖像下閱片的評價結(jié)果具有高度一致性(Kappa=0.852,P0.001),采用CT平掃結(jié)合偽彩成像閱片方式的評價結(jié)果具有較高一致性(Kappa=0.798,P0.001)。單純CT平掃閱片方式及平掃結(jié)合偽彩成像閱片方式在Ⅰ類乳腺中檢測乳腺癌的敏感性均為100%(13/13),平掃結(jié)合偽彩在Ⅱ類的乳腺中檢測腫瘤的敏感性高于平掃閱片(94.1%vs.64.7%,P0.05)。結(jié)論 CT平掃結(jié)合偽彩成像技術(shù)較之傳統(tǒng)CT閱片可以提高纖維腺體致密分布的乳腺內(nèi)癌灶的檢出率。
[Abstract]:Objective to investigate the value of pseudo-color imaging in improving the detection rate of breast cancer with plain CT scan. Methods 30 cases (60 mammary glands) of breast cancer proved by pathology were collected and read the chest CT plain mediastinal window separately by two doctors. Two weeks later, chest plain CT images combined with pseudo-color imaging were used to judge the existence of breast cancer lesions. Kappa method was used to calculate the consistency of the two physicians. Chi-square test was used to verify whether the combination of pseudo-color imaging technique could improve the detection rate of breast cancer with plain CT scan. Results in 13 cases of breast with loose distribution of fibrous glands, The average CT value of breast cancer was higher than that of normal gland [21.0 鹵13.0)HU vs(-28.1 鹵32.8)HU] P0.001a, and so was the dense distribution of fibrous glands (17 cases of class 鈪，
