【摘要】：目的探討數(shù)字鉬靶X線檢查鈣化灶對(duì)良惡性乳腺病變的診斷價(jià)值。方法回顧性分析206例患者的乳腺鉬靶X線影像資料,參照乳腺影像報(bào)告和數(shù)據(jù)系統(tǒng)(BI-RADS)評(píng)估分類,分析良惡性病變鈣化灶的形態(tài)、分布特點(diǎn)及其他陽性征象的差異。結(jié)果鈣化灶形態(tài)方面,斑點(diǎn)樣、無定形或模糊、粗糙不均質(zhì)、細(xì)小多形性、細(xì)線或細(xì)線分支狀鈣化的惡性率分別為10.96%(8/73)、39.66%(23/58)、69.23%(9/13)、78.79%(26/33)、96.56%(28/29)。鈣化灶分布方面,彌漫/散在、區(qū)域性、簇狀、段樣鈣化的惡性率分別為0、18.19%(4/22)、49.50%(50/101)、72.73%(40/55)。206例中,124例無其他陽性征象,82例伴有其他陽性征象。有無其他陽性征象患者鈣化灶形態(tài)、分布方面良惡性差異均有統(tǒng)計(jì)學(xué)意義(P均0.05)。結(jié)論乳腺鈣化的形態(tài)、分布特點(diǎn)有助于評(píng)估乳腺病變的惡性度,當(dāng)伴隨其他陽性征象時(shí),惡性度明顯提高。
[Abstract]:Objective to evaluate the diagnostic value of digital mammography in the diagnosis of benign and malignant breast lesions. Methods the mammography data of 206 patients were analyzed retrospectively and classified according to the mammography report and data system (BI-RADS). The morphology, distribution and other positive signs of calcified lesions in benign and malignant lesions were analyzed. Results the morphology of calcification foci was speckled, amorphous or fuzzy, rough and uneven, small and pleomorphic. The malignant rate of branched calcification of thin thread or fine line is 10.96 / 73 / 39.66% / 23 / 58 / 69.23% / 78.79% / 78.79% respectively. The distribution of calcification foci is diffuse / scattered, regional, cluster-shaped. The malignant rates of segmental calcification were 0.18.19 / 22 / 49.50 respectively, including 50 / 101 / 72.73 / 55 / 206 cases. Of the 206 cases, 124 cases had no other positive signs and 82 cases had other positive signs. Whether there were other positive signs or not, the shape of calcification foci was found. Conclusion the morphology and distribution characteristics of breast calcification are helpful to evaluate the malignancy of breast lesions, and the malignant degree is obviously increased when accompanied by other positive signs.
