【摘要】：目的探討第二代雙源CT在無任何心率干預(yù)狀態(tài)下行CT冠狀動(dòng)脈血管成像(CT coronary angiography,CTCA)檢查時(shí)的三種不同掃描模式的輻射劑量與圖像質(zhì)量的差異。方法收集在第二代雙源CT上行CTCA的患者350例并分成3組:A組150例用回顧性心電門控螺旋掃描,B組150例用前瞻性心電門控序列掃描,C組(心率70次/min)50例用前瞻性心電門控大螺距(Flash)掃描。A、B再根據(jù)平均心率的不同分為3個(gè)亞組:A1、B1組各50人,心率70次/min;A2、B2組各50人,70次/min≤心率≤80次/min;A3、B3組各50人,心率80次/min。各組掃描期相(R-R間期)依照A1、B1組選60%~80%、A2、B2組選30%~80%、A3、B3組選30%~50%的條件進(jìn)行。掃描結(jié)束后分別記錄每組患者所受的輻射劑量、諸冠脈血管的CT值、信噪比(signal-to-noise ratio,SNR)與對(duì)比度噪聲比(contrast-to-noise ratio,CNR)。然后由兩名診斷專家依照4分法評(píng)價(jià)諸冠脈血管的顯示情況。最后比較分析各組的輻射劑量與圖像質(zhì)量差異。結(jié)果各組輻射劑量的高低排序?yàn)锳2A1、A3、B2B1、B3C。圖像質(zhì)量方面,無論圖像質(zhì)量的主觀評(píng)價(jià)還是客觀評(píng)價(jià)各組間均沒有明顯差別。結(jié)論第二代雙源CT行CTCA檢查時(shí)在心率符合條件下應(yīng)首選FLASH掃描模式,其次再選前瞻性心電門控序列掃描模式,只有當(dāng)受檢患者存在心律不齊或是想行心功能分析亦或動(dòng)態(tài)4D顯像時(shí)才考慮回顧性心電門控螺旋掃描模式。另外,檢查中還要根據(jù)患者心率情況盡可能縮小掃描曝光窗。
