【摘要】：目的:研究絕經(jīng)后婦女椎體脆性骨折雙能X線骨密度測量結(jié)果與血清骨轉(zhuǎn)化、骨代謝指標(biāo)的關(guān)系。方法:選擇2012年4月~2015年12月期間在復(fù)旦大學(xué)附屬上海市第五人民醫(yī)院接受DXA檢查的158例絕經(jīng)后婦女,根據(jù)骨密度測量結(jié)果分為骨質(zhì)疏松組、骨量減少組、骨質(zhì)正常組,根據(jù)胸腰椎正側(cè)位片結(jié)果分為無椎體骨折組、胸椎骨折組、腰椎骨折組、胸腰椎骨折組,采集血清并測定骨轉(zhuǎn)化和骨代謝指標(biāo)。結(jié)果:胸椎骨折、胸腰椎骨折受試者的股骨頸、髖部、腰椎L1~4骨密度顯著低于無椎體骨折的受試者,腰椎骨折受試者的股骨頸、髖部、腰椎L1~4骨密度與無椎體骨折的受試者無顯著性差異;骨質(zhì)疏松組、骨量減少組絕經(jīng)后婦女血清中PINP、ICTP、CTX、TRACP-5b、MMP13、OPG、OPN含量顯著高于骨量正常組,25(OH)D、BGP、ON含量顯著低于骨量正常組;骨質(zhì)疏松組絕經(jīng)后婦女血清中PINP、ICTP、CTX、TRACP-5b、MMP13、OPG、OPN含量顯著高于骨量減少組,25(OH)D、BGP、ON含量顯著低于骨量減少組。結(jié)論:雙能X線骨密度測定對(duì)絕經(jīng)后婦女胸椎脆性骨折和胸腰椎脆性骨折的預(yù)測價(jià)值明確且與骨轉(zhuǎn)化、骨代謝的改變密切相關(guān)。
[Abstract]:Objective: To study the postmenopausal women with vertebral fracture dual energy bone density measurements and X-ray conversion relationship between serum bone, bone metabolism index. Methods: April 2012 ~2015 year in December at the Fudan University affiliated Fifth People's Hospital of Shanghai, received DXA examination of 158 cases of postmenopausal women, according to the measurement of bone mineral density results were divided into osteoporosis group osteopenia group, normal group, osteoporosis, according to thoracic and lumbar radiographs were divided into non vertebral fracture group, thoracic fracture group, lumbar fracture group, thoracolumbar fracture group, serum and determination of bone turnover and bone metabolism index. Results: thoracic vertebral fracture, the subjects of the femoral neck, thoracic and lumbar fractures hip bone density in L1~4 was significantly lower than that of the subjects without vertebral fracture, lumbar fracture subjects of femoral neck, hip, there was no significant difference in bone density in L1~4 and non vertebral fracture subjects; osteoporosis group, bone mass reduction Small group of postmenopausal women in serum PINP, ICTP, CTX, TRACP-5b, MMP13, OPG, OPN were significantly higher than that in normal bone mass group, 25 (OH) D, BGP, ON were significantly lower than normal bone mass group; osteoporosis group of postmenopausal women in serum PINP, ICTP, CTX, TRACP-5b, MMP13, OPG, OPN the content was significantly higher than that of osteopenia group, 25 (OH) D, BGP, ON were significantly lower in osteopenia group. Conclusion: dual energy X-ray bone density determination of predictive value of postmenopausal women with thoracic and thoracolumbar fracture of brittle fracture of brittle bone and clear and transformation, is closely related to bone metabolism changes.
