【摘要】：目的探討藥物流產(chǎn)中縮短米非司酮用藥時(shí)間的可行性。方法將80例因非意愿妊娠自愿要求終止早孕的女性隨機(jī)分為4組:對照組(直接負(fù)壓吸宮術(shù))、用藥后12 h組、24 h組、48 h組,3個(gè)用藥組頓服米非司酮150 mg,于不同時(shí)間行負(fù)壓吸引術(shù),留取各組蛻膜組織。應(yīng)用光學(xué)顯微鏡、透射電子顯微鏡觀察4組蛻膜組織的形態(tài)學(xué)改變,應(yīng)用免疫組織化學(xué)方法和計(jì)算機(jī)圖像分析系統(tǒng)檢測四組蛻膜組織中類固醇受體的表達(dá)。結(jié)果 3個(gè)用藥組蛻膜細(xì)胞變性、壞死,間質(zhì)水腫、出血,毛細(xì)血管擴(kuò)張充血。對照組蛻膜組織中雌激素受體(ER)和孕激素受體(PR)陽性染色位于大部分蛻膜細(xì)胞及少數(shù)腺上皮細(xì)胞,用藥后12 h組、24 h組、48 h組蛋白表達(dá)均低于對照組,差異有統(tǒng)計(jì)學(xué)意義(P0.05),而這3組之間無統(tǒng)計(jì)學(xué)差異(P0.05)。結(jié)論米非司酮用藥后12 h蛻膜組織變性壞死,蛋白表達(dá)受到抑制,為臨床縮短藥物流產(chǎn)用藥時(shí)間提供了一定的理論參數(shù)。
[Abstract]:Objective to study the feasibility of shortening the time of mifepristone in drug abortion. Methods Eighty women who voluntarily requested termination of early pregnancy due to unwanted pregnancy were randomly divided into four groups: control group (direct negative pressure aspiration of uterus), 12 h group, 24 h group, 48 h group, and 3 drug groups taking mifepristone 150 mg, at different time Between negative pressure suction, The decidual tissue was retained in each group. The morphologic changes of decidua tissues were observed by optical microscope and transmission electron microscope. The expression of steroid receptors in decidua tissues was detected by immunohistochemical method and computer image analysis system. Results in the three groups, decidual cell degeneration, necrosis, interstitial edema, hemorrhage, capillary dilatation and hyperemia. The positive staining of estrogen receptor (ERR) and progesterone receptor (PRP) in decidua of control group was found in most decidual cells and a few glandular epithelial cells. The difference was statistically significant (P 0.05), but there was no significant difference among the three groups (P 0.05). Conclusion the degeneration and necrosis of decidua tissue and the inhibition of protein expression at 12 h after mifepristone treatment may provide some theoretical parameters for shortening the time of drug abortion.
