【摘要】：早產(chǎn)兒營(yíng)養(yǎng)補(bǔ)給的目標(biāo)是使早產(chǎn)兒能夠達(dá)到同胎齡健康胎兒的正常生長(zhǎng)速率,并且在器官生長(zhǎng)、組織成分、以及細(xì)胞數(shù)量和結(jié)構(gòu)方面也能達(dá)到健康胎兒水平。營(yíng)養(yǎng)物質(zhì)的數(shù)量與質(zhì)量對(duì)于早產(chǎn)兒的正常生長(zhǎng)發(fā)育、包括神經(jīng)系統(tǒng)發(fā)育是至關(guān)重要的�；緺I(yíng)養(yǎng)素的供給不足不僅會(huì)造成生長(zhǎng)受限,而且會(huì)增加病死率或影響神經(jīng)發(fā)育結(jié)局。早產(chǎn)兒在NICU住院期間的生長(zhǎng)速率在神經(jīng)發(fā)育及整個(gè)人體發(fā)育的結(jié)局中發(fā)揮了重要作用。盡管對(duì)于最佳營(yíng)養(yǎng)的需求非常明顯,但早產(chǎn)兒生長(zhǎng)落后的例子比比皆是。因此,根據(jù)總的能量及蛋白質(zhì)需求以及個(gè)體成分如氨基酸、碳水化合物及脂肪,甚至細(xì)化到氧氣的需求來(lái)優(yōu)化早產(chǎn)兒的營(yíng)養(yǎng)是非常必要的。該綜述闡述了早產(chǎn)兒科學(xué)合理的營(yíng)養(yǎng)需求、具有實(shí)用性的營(yíng)養(yǎng)指南以及早產(chǎn)兒靜脈營(yíng)養(yǎng)、腸內(nèi)營(yíng)養(yǎng)的方法步驟。包括氨基酸在內(nèi)的靜脈營(yíng)養(yǎng),應(yīng)該從一出生便按照相應(yīng)胎齡所適合的速率開(kāi)始補(bǔ)充。腸內(nèi)營(yíng)養(yǎng)則應(yīng)在出生后盡早開(kāi)始,首選母親的初乳和牛奶。腸內(nèi)營(yíng)養(yǎng)應(yīng)根據(jù)熱能需要開(kāi)始建立并在能耐受的范圍內(nèi)快速增加,保持營(yíng)養(yǎng)攝入在推薦速率的同時(shí)相應(yīng)地減少靜脈營(yíng)養(yǎng)攝入量。制定一個(gè)合理的喂養(yǎng)方案對(duì)于改善營(yíng)養(yǎng)狀況及相關(guān)的轉(zhuǎn)歸是重要的。什么是早產(chǎn)兒的最佳營(yíng)養(yǎng)和生長(zhǎng)速率尚有待于進(jìn)一步的研究來(lái)確定,這種最佳營(yíng)養(yǎng)不僅能使早產(chǎn)兒神經(jīng)認(rèn)知得到最充分的發(fā)育,同時(shí)還要最大程度地限制其遠(yuǎn)期發(fā)生慢性疾病的風(fēng)險(xiǎn)。

It is necessary to refine the oxygen requirement to optimize the nutrition of premature infants. This review describes the scientific and reasonable nutritional requirements of premature infants, practical nutrition guidelines and intravenous nutrition of premature infants. Parenteral nutrition. Parenteral nutrition, including amino acids, should be replenished at a rate appropriate to the gestational age at birth, and enteral nutrition should begin as early as possible after birth. First choice for mother's colostrum and milk. Enteral nutrition should be established in accordance with thermal energy needs and be rapidly increased within the tolerance range. Maintain nutritional intake at a recommended rate and reduce intravenous intake accordingly. It is important to develop a reasonable feeding plan to improve nutritional status and related outcomes.
