【摘要】：目的:探討一款經(jīng)筆者改良后兼具灌注、娩核、抽吸等多種功能的雙腔管在小切口白內(nèi)障手術(shù)中的應(yīng)用價值。方法:應(yīng)用改良雙腔管,對188例188眼進行白內(nèi)障摘除手術(shù),觀察灌注下撕囊、一次性娩核、水灌注下人工晶體植入等操作的成功率,以及手術(shù)后效果和相關(guān)并發(fā)癥的發(fā)生情況。結(jié)果:應(yīng)用改良雙腔管進行小切口白內(nèi)障手術(shù),一次性娩核成功率為89%;水灌注下環(huán)形撕囊成功率為93.6%;水灌注下人工晶體植入成功率為95.7%,手術(shù)并發(fā)癥主要為后囊破裂(2.7%)、角膜水腫(14.9%)、人工晶體前膜形成(0.1%)。術(shù)后第1、7天視力在0.3以上者分別為141眼(75%)和178眼(94.9%);術(shù)后3個月角膜散光度為(0.71±0.59)D。結(jié)論:自行改良的雙腔管具有多功能、操作簡便、易于掌握等優(yōu)點,手術(shù)成功率高,術(shù)后效果好,是小切口白內(nèi)障手術(shù)中理想的手術(shù)器械。
[Abstract]:Objective: to investigate the application value of a modified dual cavity tube with multiple functions of perfusion, nucleus delivery and aspiration in small incision cataract surgery. Methods: 188 eyes of 188 cases with cataract were treated with modified double lumen tube. The successful rate of operation such as capsulorhexis, one-off nucleus delivery and intraocular lens implantation under water perfusion were observed, as well as the results of the operation and the occurrence of related complications. Results: the success rate of one-off nucleus delivery was 89 and the success rate of circular capsulorhexis was 93.6 under water perfusion. The successful rate of intraocular lens implantation was 95.7%. The main complications were posterior capsule rupture (2.7%), corneal edema (14.9%) and intraocular lens anterior membrane formation (0.1%). The visual acuity was better than 0.3 in 141 eyes (75%) and 178 eyes (94.9%) on the 1st day after operation, and the corneal astigmatism was (0.71 鹵0.59) D at 3 months after operation. Conclusion: the self-modified double-lumen tube has the advantages of multifunction, simple operation, easy to master, high success rate and good postoperative effect. It is an ideal instrument for small incision cataract surgery.
