【摘要】：目的探討懸雍垂腭咽成形術(shù)(UPPP)對(duì)阻塞性睡眠呼吸暫停低通氣綜合征(OSAHS)患者發(fā)聲功能和口鼻共鳴功能的影響。方法對(duì)30例確診為OSAHS的成年男性患者行UPPP手術(shù),分別在術(shù)前、術(shù)后1周及術(shù)后3月時(shí)通過(guò)多維嗓音分析軟件、線性預(yù)測(cè)譜、鼻流計(jì)檢測(cè)等方法,客觀評(píng)估其發(fā)聲功能和口鼻共鳴功能。結(jié)果術(shù)前、術(shù)后1周及術(shù)后3月患者的嗓音基頻(F0)、頻率微擾(jitter)、振幅微擾(shimmer)、諧噪比(NHR)差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05);術(shù)后1周時(shí)元音/a/、/i/、/u/的F1、F2與術(shù)前差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05);術(shù)后3月時(shí)元音/i/的F1、F2及元音/u/的F1與術(shù)前差異仍無(wú)統(tǒng)計(jì)學(xué)意義(P0.05),但元音/a/的F1、F2及元音/u/的F2較術(shù)前升高,差異有統(tǒng)計(jì)學(xué)意義(P0.05);術(shù)后1周時(shí)非鼻音句、口鼻音句、鼻音句的鼻音率與術(shù)前差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05);術(shù)后3月時(shí)非鼻音句、口鼻音句的鼻音率較術(shù)前差異仍無(wú)統(tǒng)計(jì)學(xué)意義(P0.05),但鼻音句的鼻音率較術(shù)前略有升高,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論 UPPP手術(shù)對(duì)OSAHS患者的發(fā)聲功能無(wú)明顯影響,但對(duì)口鼻共鳴功能有輕微影響;對(duì)職業(yè)用嗓者行UPPP手術(shù)需慎重。
[Abstract]:Objective to investigate the effects of uvulopalatopharyngoplasty (UPPP) on vocal function and oral and nasal resonance in patients with obstructive sleep apnea hypopnea syndrome (OSAHS). Methods UPPP was performed on 30 adult male patients diagnosed as OSAHS. The data were analyzed by multi-dimensional voice analysis software, linear predictive spectrum and nasal flow meter before operation, 1 week after operation and 3 months after operation. Objective evaluation of vocal function and oral and nasal resonance function. Results there was no significant difference in fundamental voice frequency (F0), frequency perturbation (jitter), amplitude, (shimmer), harmonic noise ratio (NHR) between patients before operation, 1 week after operation and 3 months after operation (P0.05). At 1 week after operation, there was no significant difference between the vowel / a / r / I / F _ 1 / F _ 2 and preoperative (P 0.05). There was no significant difference in F _ 1 F _ 2 and F _ 1 / u / F _ 1 between the vowel / I / F _ 2 and vowel / u / vowels 3 months after operation (P0.05), but the vowel / a / F _ 1 F _ 2 and vowel / u / F _ 2 were higher than those before operation. The difference was statistically significant (P0.05). At 1 week after operation, there was no significant difference in nasal rate of non-nasal sentence, oral nasal sentence and nasal sentence between before and after operation (P0.05). At 3 months after operation, the nasal rate of non-nasal sentence and oral nasal sentence had no statistical significance (P0.05), but the nasal sound rate of nasal sentence was slightly higher than that of preoperative (P0.05). Conclusion UPPP operation has no significant effect on the vocal function of OSAHS patients, but has a slight effect on the oral and nasal resonance function, and UPPP surgery for professional voice patients should be careful.
