【摘要】：目的系統評價白內障囊外摘除術與超聲乳化吸出術對角膜內皮細胞及角膜中央厚度的影響。方法按Cochrane系統評價方法,計算機檢索MEDLINE (1966～2010.11). EMbase (1980-2010.11)、Cochrane圖書館(2008年3期)和中國生物醫(yī)學文獻數據庫(CBM,1979～2010.11),并手工檢索相關會議文獻。文獻語種限制為中英文。收集所有相關隨機對照試驗,采用RevMan 5.0軟件進行Meta分析。結果共納入4篇隨機對照試驗,包括704例患者(共704只患眼),隨訪1～12個月。Meta分析結果顯示：白內障囊外摘除術與白內障超聲乳化吸出術在術后角膜內皮細胞計數方面[WMD=55.26,95%CI(0.79,109.73)],P=0.05,及在術后角膜中央厚度方面[WMD=28.21,95%CI(12.29,44.13)],P=0.0005,有差異。結論與白內障囊外摘除術相比,白內障超聲乳化吸出術術后角膜內皮細胞計數減少更多,角膜中央厚度增加更多。因本系統評價納入研究病例數較少,上述結論需更多設計嚴謹大樣本隨機對照試驗加以驗證。
[Abstract]:Objective to evaluate the effects of extracapsular cataract extraction and phacoemulsification on corneal endothelium and central corneal thickness. Methods according to the evaluation method of Cochrane system, MEDLINE (1966 / 2010.11) was searched by computer. EMbase (1980-2010.11) includes the Cochrane Library (3 issues 2008) and the Chinese Biomedical Literature Database (CBM,1979~2010.11). The language of the literature is restricted to Chinese and English. All the related randomized controlled trials were collected and Meta analysis was carried out with RevMan 5.0 software. Results A total of 4 randomized controlled trials were conducted. In 704 patients (704 eyes), the results of meta-analysis showed that extracapsular cataract extraction and phacoemulsification [WMD=55.26,95%CI _ (0.79109.73)] P0. 05, and corneal central thickness after cataract extraction and phacoemulsification. The degree of WMD=28.21,95%CI (12.29 ~ 44.13) was 0.0005. Conclusion compared with extracapsular cataract extraction, the number of corneal endothelium and the central thickness of cornea after phacoemulsification were decreased more and more. Due to the small number of cases included in this systematic evaluation, the above conclusions need to be verified by more well-designed randomized controlled trials with large samples.
