【摘要】：目的:探討鼻腔鼻竇畸胎癌肉瘤(SNTCS)的臨床病理學(xué)特征、免疫學(xué)表型、診斷及鑒別診斷。方法:分析1例SNTCS的臨床表現(xiàn),對(duì)標(biāo)本進(jìn)行病理學(xué)和免疫組織化學(xué)(SP法)染色觀察并復(fù)習(xí)相關(guān)文獻(xiàn)。結(jié)果:腫瘤組織由3個(gè)胚層構(gòu)成,有外胚層的成熟和未成熟的鱗狀細(xì)胞巢、神經(jīng)上皮細(xì)胞團(tuán)及嗅母細(xì)胞樣細(xì)胞,中胚層的肉瘤樣成分和骨組織,以及內(nèi)胚層的呼吸道上皮及腺管樣結(jié)構(gòu),部分為腺癌;其中幼稚的透明細(xì)胞鱗狀細(xì)胞巢非常具有特征性。此外,尚可見(jiàn)彌漫大片胞質(zhì)豐富的淡染區(qū)及胞質(zhì)較少的深染區(qū),二者疏密有間。免疫組織化學(xué)染色顯示不同的胚層對(duì)應(yīng)相應(yīng)的免疫標(biāo)記,鱗狀上皮、腺上皮和呼吸道上皮CK、EMA等陽(yáng)性,神經(jīng)上皮細(xì)胞團(tuán)及嗅母細(xì)胞樣細(xì)胞S-100、NSE、Syn等陽(yáng)性,肉瘤樣區(qū)Vim陽(yáng)性,淡染區(qū)Vim、CD99及CK等不同程度陽(yáng)性,深染區(qū)NSE和GFAP陽(yáng)性。結(jié)論:SNTCS是具有畸胎瘤和癌肉瘤特征的罕見(jiàn)惡性腫瘤,具有特征性的組織病理學(xué)和免疫組織化學(xué)染色特點(diǎn),應(yīng)多取材多切片,以免誤診和漏診。
[Abstract]:Objective: to investigate the clinicopathological features, immunophenotype, diagnosis and differential diagnosis of teratosarcoma of nasal cavity and paranasal sinuses (SNTCS). Methods: the clinical manifestations of one case of SNTCS were analyzed. The pathological and immunohistochemical (SP) staining methods were used to observe and review the relevant literature. Results: the tumor tissue was composed of three layers, including mature and immature squamous cell nests, neuroepithelial cell clusters and olfactory blast-like cells, sarcomatoid components and bone tissue of mesoderm. The respiratory epithelium and glandular tubules of the endoderm are partly adenocarcinoma, and the nests of the squamous cells of the puerile transparent cells are very characteristic. In addition, we can see a large area of diffuse cytoplasm rich light stained area and less cytoplasm deep stained area. Immunohistochemical staining showed that the corresponding immunological markers were found in different embryonic layers, such as CK,EMA positive in squamous epithelium, glandular epithelium and respiratory tract epithelium, S-100NSE-Syn positive in neuroepithelial cell clusters and olfactory mother cell-like cells, and Vim positive in sarcomatoid region. Vim,CD99 and CK were positive in light stained area and NSE and GFAP in deep stained area. Conclusion SNTCS is a rare malignant tumor with the characteristics of teratoma and carcinosarcoma. It has the characteristics of histopathology and immunohistochemical staining. We should take more materials and sections to avoid misdiagnosis and missed diagnosis.
