【摘要】： 目的 觀察并評價散瞳飲水暗室低頭激發(fā)試驗(Mydriasis water-drinking dark-room lower-head-position provocative test, MWDLPT, PT)在可疑青光眼(Glaucoma suspects, GS)中的臨床應用。 方法 收集2004年2月～2010年1月間于中國醫(yī)大一院眼科門診青光眼組就診的GS共37例(74眼),年齡15～78歲(平均年齡50.7±19.3歲)。非同日分別行PT和日間眼壓(Daytime intraocular pressure, DIOP)測量等輔助檢查,分別記錄兩種方法測得的眼壓波動值(AIOP)和眼壓峰值(Peak IOP),兩種方法均以AIOP≥8mmHg為陽性,比較兩種方法參數間的關系。分別比較PT結果與周邊前房深度(Peripheral anterior chamber depth, PACD)和年齡(Age)間的關系。采用X2檢驗及線性回歸分析進行統(tǒng)計學分析,以P0.05表示有統(tǒng)計學意義。 結果 PT陽性者為21例(37眼)。無1例出現(xiàn)嚴重的并發(fā)癥(如明顯的眼痛,頭痛,視力下降及惡心嘔吐等)。所有PT試驗對象的眼壓在試驗后1-2小時內降至基線水平,部分病例應用了縮瞳劑。PT與DIOP兩種方法的Peak IOP成顯著的正相關關系,且PT法的陽性率顯著高于DIOP法。在PACD≤1/4角膜厚度(Corneal thickness, CT)組PT的陽性率顯著高于PACD＞1/4CT組。PT法△IOP與年齡無相關性。 結論 PT可作為一種新的可疑青光眼的激發(fā)試驗,在對眼壓波動情況的檢查上優(yōu)于DIOP法,可能更適用于閉角型青光眼(Angle-closure glaucoma, ACG),且不受年齡影響,應用范圍較廣泛,是一種相對安全有效的檢查方法。
