【摘要】：目的：探討上瞼提肌縮短聯(lián)合部分瞼板切除矯正中、重度上瞼下垂的方法和療效。資料和方法：對13例（2012年7月～2014年2月）（17只眼）中、重度上瞼下垂患者行上瞼提肌縮短聯(lián)合部分瞼板切除。其中男性4例，女性9例；單側(cè)9例，雙側(cè)4例。年齡4～76歲，平均年齡22歲。術(shù)后隨訪3～12個月（平均6個月），，采用以下指標(biāo)進(jìn)行檢查、評價及分析手術(shù)療效：⑴上瞼緣的位置⑵上瞼緣弧度⑶上瞼緣高度對稱度⑷眼瞼閉合程度⑸雙重瞼皺璧⑹上瞼遲落現(xiàn)象⑺額肌活動度⑻患者滿意度⑼暴露性角膜炎。 結(jié)果：本組13例（17只眼）中、重度上瞼下垂均矯正滿意（外觀滿意，弧度自然）。早期均有不同程度的眼瞼閉合不全的情況，一般術(shù)后3～6個月內(nèi)完全恢復(fù)。除1例（1只眼）在用力狀態(tài)下無法完全閉合外，大部分眼瞼能自然閉合。其中1例因術(shù)后復(fù)發(fā)，重瞼皺璧變淺、消失，其余均明顯。有2例（2只眼）過矯，有1例（1只眼）可以觀察到明顯的上瞼遲滯現(xiàn)象，余均獲得良好效果。均未出現(xiàn)暴露性角膜炎，且上瞼緣弧度均基本滿意。 結(jié)論：上瞼提肌縮短聯(lián)合部分瞼板切除術(shù)，此手術(shù)操作簡單，并發(fā)癥少、療效確切。能達(dá)到既符合眼瞼生理功能，又改善美觀的效果，是治療該疾病的比較滿意術(shù)式。
[Abstract]:Objective: to investigate the method and effect of shortening levator palpebrae combined with partial blepharoplasty for correction of moderate and severe blepharoptosis. Materials and methods: 13 cases (July 2012 to February 2014) (17 eyes) with severe blepharoptosis were treated with levator palpebrae shortening combined with partial tarsal plate resection. Among them, 4 cases were male, 9 cases were female, 9 cases were unilateral and 4 cases were bilateral. The average age was 22 years. The patients were followed up for 3 to 12 months (mean 6 months). To evaluate and analyze the effect of operation on the position of upper palpebral margin: 2 upper eyelid margin radians 3 upper eyelid edge height symmetry 4 eyelid closure degree 5 double eyelid fold 6 palpebral tardiness 7 frontalis movement degree 8 patient satisfaction with 9 exposed keratitis. Results: in 13 cases (17 eyes), the correction of severe blepharoptosis was satisfactory (satisfactory appearance and natural radians). There were different degree of eyelid incompetence in early stage, which generally recovered within 3 ~ 6 months after operation. Most eyelids could be closed naturally except in one case (1 eye) which could not be closed completely under the condition of force. In one case, the double eyelid wrinkle became shallow and disappeared because of postoperative recurrence, and the others were obvious. Two cases (2 eyes) were overcorrected and 1 case (1 eye) could observe obvious hysteretic phenomenon of upper eyelid. No exposed keratitis was found, and the upper eyelid margin radians were satisfactory. Conclusion: levator palpebrae shortening combined with partial blepharoidectomy is simple, less complication and effective. It can not only accord with the physiological function of eyelid, but also improve the aesthetic effect.
