PTEN、MMP-9、Survivin和Ki67在視網(wǎng)膜母細(xì)胞瘤中的表達(dá)和意義

【摘要】: 目的探討PTEN、MMP-9、Survivin和Ki67在視網(wǎng)膜母細(xì)胞瘤中的表達(dá)及其與臨床病理因素的關(guān)系 方法采用免疫組織化學(xué)方法檢測(cè)41例視網(wǎng)膜母細(xì)胞瘤切片和10例正常視網(wǎng)膜組織中PTEN、MMP-9、Survivin和Ki67的表達(dá)情況,分析四者分別表達(dá)及共表達(dá)與視網(wǎng)膜母細(xì)胞瘤患者性別、臨床分期、視神經(jīng)受侵犯情況和分化程度的關(guān)系。所有數(shù)據(jù)應(yīng)用SPSS13.0軟件進(jìn)行統(tǒng)計(jì)。 結(jié)果 1.免疫組織化學(xué)染色結(jié)果表明:視網(wǎng)膜母細(xì)胞瘤中PTEN、MMP-9、Survivin和Ki67的陽性表達(dá)率分別為68.3%,60.9%,63.4%和43.9%, PTEN表達(dá)明顯降低,MMP-9、Survivin和Ki67表達(dá)明顯增高,與對(duì)照組比較差異具有統(tǒng)計(jì)學(xué)意義(P0.01)。經(jīng)Pearsonχ2檢驗(yàn)顯示,不同性別之間PTEN、MMP-9、survivin和Ki-67表達(dá)均無統(tǒng)計(jì)學(xué)差異(p0.05)。 2.采用Fisher's Exact Test檢驗(yàn)顯示,眼內(nèi)生長(zhǎng)期PTEN表達(dá)陽性率(90.0%)和眼內(nèi)壓增高期陽性率(84.2%)相比,差異無統(tǒng)計(jì)學(xué)意義(P0.05);眼內(nèi)生長(zhǎng)期與眼外擴(kuò)展期PTEN表達(dá)陽性率比較,差異有顯著統(tǒng)計(jì)學(xué)意義(P0.05);眼內(nèi)壓增高期與眼外擴(kuò)展期比較,差異有統(tǒng)計(jì)學(xué)意義(P 0.05)。PTEN在伴有視神經(jīng)浸潤(rùn)的RB組織中的表達(dá)陽性率明顯低于不伴視神經(jīng)浸潤(rùn)者,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。在RB組織分級(jí)之間,分化型RB組織中的PTEN蛋白表達(dá)陽性率明顯高于未分化型,差異有統(tǒng)計(jì)學(xué)意義(P0、05)。眼內(nèi)生長(zhǎng)期,MMP-9表達(dá)陽性率(40.0%)和眼內(nèi)壓增高期陽性率(52.6%)相比,差異無統(tǒng)計(jì)學(xué)意義(P0.05);眼內(nèi)生長(zhǎng)期與眼外擴(kuò)展期比較,差異有顯著統(tǒng)計(jì)學(xué)意義(P0.05);眼內(nèi)壓增高期與眼外擴(kuò)展期比較,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。MMP-9在伴有視神經(jīng)浸潤(rùn)的RB組織中,表達(dá)陽性率明顯高于不伴視神經(jīng)浸潤(rùn)者,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。在RB組織分級(jí)之間,分化型RB組織中MMP-9表達(dá)陽性率明顯低于未分化型,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。survivin的表達(dá)陽性率與患者的性別、臨床分期、組織學(xué)分型差異無統(tǒng)計(jì)學(xué)意義(P0.05)。眼內(nèi)生長(zhǎng)期和眼內(nèi)壓增高期Ki67陽性表達(dá)率明顯低于眼外擴(kuò)展期,差異具有統(tǒng)計(jì)學(xué)意義(P0、05)。Ki67在伴有視神經(jīng)浸潤(rùn)的RB組織中,表達(dá)陽性率明顯高于不伴視神經(jīng)浸潤(rùn)者,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。在RB組織分級(jí)之間,分化型RB組織中Ki67表達(dá)陽性率明顯低于未分化型,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。 經(jīng)spearman等級(jí)相關(guān)分析,性別與MMP-9、survivin、Ki-67共表達(dá)的相關(guān)性不存在統(tǒng)計(jì)學(xué)差異(rs=-0.13,p0.05)。臨床分期與MMP-9、survivin、Ki-67共表達(dá)存在正相關(guān)(rs=0.434,p<0.05)。視神受經(jīng)侵犯程度與MMP-9、survivin、Ki-67共表達(dá)存在正相關(guān)(rs=0.470,p0.05。組織分化程度與MMP-9、survivin、Ki-67共表達(dá)存在負(fù)相關(guān)(rs=-0.566,p0.01)。PTEN表達(dá)與MMP-9、survivin、Ki-67共表達(dá)存在負(fù)相關(guān)(rs=-0.542,p0.01)。結(jié)論P(yáng)TEN在視網(wǎng)膜母細(xì)胞瘤組織中的陽性表達(dá)率顯著降低,MMP-9、Survivin和Ki-67在視網(wǎng)膜母細(xì)胞瘤組織中的陽性表達(dá)率顯著高于正常視網(wǎng)膜組織,四者單獨(dú)表達(dá)或共表達(dá)與視網(wǎng)膜母細(xì)胞瘤的發(fā)生、侵襲和轉(zhuǎn)移有關(guān)。
[Abstract]:Objective to investigate the expression of survivin and Ki67 in retinoblastoma and their relationship with clinicopathological factors. Methods Immunohistochemical method was used to detect 41 cases of retinoblastoma sections and 10 cases of normal visual reticulum. Expression of survivin and Ki67 in membrane tissue To analyze the relationship between the expression and coexpression of retinoblastoma and sex, clinical stage, optic nerve invasion and degree of differentiation. All data are counted by SPSS13.0 software. Result 1. The results of immunohistochemical staining showed that the positive rates of survivin and Ki67 in retinoblastoma were 68.3% and 43.9%, respectively. The expression of PTEN significantly decreased the expression of MMP-9, survivin and Ki67 in retinoblastoma, which was significantly higher than that in control group (P0.01). Pearson 蠂 2 test showed that there was no significant difference in the expression of MMP-9, survivin and Ki-67 between different genders (p0.05). The positive rate of PTEN expression in intraocular growth stage (90.0%) was not significantly higher than that in intraocular hypertension stage (84.2%) (P0.05), and the positive rate of PTEN expression in intraocular growth phase was higher than that in extraocular expansion stage (P 0.05). The difference was statistically significant (P0.05), the positive rate of PTEN expression in RB with optic nerve infiltration was significantly lower than that in patients without optic nerve infiltration. The difference was statistically significant (P0.05). The positive rate of PTEN protein expression in differentiated RB tissues was significantly higher than that in undifferentiated RB tissues (P0. 05). The positive rate of MMP-9 expression in intraocular growth stage (40.0%) was not significantly different from that in intraocular pressure (52.6%) period (P0.05), and there was significant difference between intraocular growth phase and extraocular dilatation stage (P0.05). The positive rate of MMP-9 in RB with optic nerve invasion was significantly higher than that without optic nerve infiltration (P0.05). The positive rate of MMP-9 expression in differentiated RB tissues was significantly lower than that in undifferentiated RB tissues (P0.05). There was no significant difference between the positive rate of MMP-9 expression and gender, clinical stage and histological classification (P0.05). The positive expression rate of Ki67 in the stage of intraocular growth and increased intraocular pressure was significantly lower than that in the stage of extraocular expansion (P005) .Ki67 was significantly higher in RB tissues with optic nerve infiltration than in those without optic nerve infiltration. The difference was statistically significant (P0.05). The positive rate of Ki67 expression in differentiated RB was significantly lower than that in undifferentiated RB (P0.05). By spearman grade correlation analysis, there was no statistical difference between sex and MMP-9 survivin Ki-67 co-expression (rs-0.13p0.05). There was a positive correlation between the clinical stage and the expression of MMP-9, survivin and Ki-67 (r sn 0.434, p < 0.05). There was a positive correlation between the degree of invasion and the co-expression of MMP-9 and survivin Ki-67 (r s0. 470 + p0. 05). There was a negative correlation between the degree of tissue differentiation and the co-expression of MMP-9, survivin and Ki-67 (rstl -0.566p0.01). The expression of PTEN was negatively correlated with the co-expression of MMP-9 survivin Ki-67 (rs-0.542p0.01). Conclusion the positive expression rate of PTEN in retinoblastoma is significantly lower than that in normal retinoma. Expression alone or coexpression in four patients was related to the occurrence, invasion and metastasis of retinoblastoma.
