恒定性外斜視弱側眼外肌形態(tài)改變及MyoD、Pax7表達研究

【摘要】:目的:觀察恒定性外斜視弱側眼外肌形態(tài)改變及MyoD和Pax7表達變化,探討MyoD、Pax7在恒定性外斜視發(fā)生、發(fā)展中的作用。 方法:收集2009年12月至2010年9月在青島大學醫(yī)學院附屬醫(yī)院眼科住院手術的恒定性外斜視患者24例作為實驗組,分為兒童外斜視組、成人外斜視組各12例,另選同期角膜移植供體的眼外肌10例作為成人對照組。實驗組手術截取的內直肌及對照組的內直肌固定切片后行HE染色,在光鏡下觀察肌纖維的大小、形態(tài)、數(shù)量、走形方向等,統(tǒng)計肌纖維數(shù)量及肌細胞橫截面積。免疫組化法測量MyoD和Pax7的平均光密度值。 結果:恒定性外斜視弱側眼外肌肌纖維數(shù)量減少,大小及形態(tài)不一,走形方向不一致,細胞間質成分增多。成人外斜視組肌纖維數(shù)量為18.77±2.91個,成人對照組為32.53±4.75個,兩者差異具有統(tǒng)計學意義(P0.01)。而兒童外斜視組為20.81±4.27個,與成人外斜視組比較,差異無統(tǒng)計學意義(P0.05)。肌纖維橫截面積:成人外斜視組為261.76±35.68μm2,成人對照組708.9±62.9μm2,兩者差異具有統(tǒng)計學意義(P0.01)。而兒童外斜視組平均肌纖維橫截面積為282.46±25.22μm2,與成人外斜視組比較,差異無統(tǒng)計學意義(P0.05)。成人外斜視組MyoD的平均光密度值(0.011±0.006)、Pax7的值(0.013±0.005)均較成人對照組(0.028±0.0008,0.027±0.002)明顯減少(P0.01)。兒童外斜視組MyoD值(0.021±0.024)、Pax7的值(0.014±0.004)分別與成人外斜視組比較,差異均無統(tǒng)計學意義(P0.05)。 結論:恒定性外斜視弱側眼外肌肌纖維發(fā)生了萎縮、減少為主的形態(tài)學改變,以及MyoD, Pax7的表達下降,這些改變可能與共同性外斜視斜視的發(fā)生及發(fā)展有關。
[Abstract]:Aim: to observe the morphological changes of weak lateral extraocular muscles and the expression of MyoD and Pax7 in constant exotropia, and to explore the role of MyoD Pax7 in the occurrence and development of constant exotropia. Methods: from December 2009 to September 2010, 24 patients with constant exotropia who were operated on in the affiliated Hospital of Qingdao University Medical College as experimental group were divided into two groups: child exotropia group (n = 12), adult exotropia group (n = 12) and adult exotropia group (n = 12). Another 10 cases of corneal transplantation donor were selected as the adult control group. The internal rectus muscle of the experimental group and the internal rectus muscle of the control group were fixed by HE staining. The size, shape, quantity and direction of the muscle fibers were observed under light microscope, and the number of muscle fibers and the cross sectional area of muscle cells were counted. The mean optical density of MyoD and Pax7 was measured by immunohistochemical method. Results: the number of muscle fibers in the weak lateral extraocular muscle of constant exotropia was decreased, the size and shape of the muscle fibers were different, the direction of deviation was different, and the content of the interstitial cells was increased. The number of muscle fibers in adult exotropia group was 18.77 鹵2.91, and that in adult control group was 32.53 鹵4.75. The difference was statistically significant (P0.01). The number of children with exotropia was 20.81 鹵4.27, which was not significantly different from that of adult exotropia (P0.05). The cross sectional area of muscle fiber was 261.76 鹵35.68 渭 m2 in adult exotropia group and 708.9 鹵62.9 渭 m2 in adult control group. The difference was statistically significant (P0.01). The mean cross sectional area of muscle fiber in children with exotropia was 282.46 鹵25.22 渭 m ~ (2), which was not significantly different from that in adult exotropia group (P0.05). The mean optical density of MyoD in adult exotropia group (0.011 鹵0.006) was (0.013 鹵0.005) lower than that in adult control group (0.028 鹵0.0008 鹵0.027 鹵0.002). The values of MyoD (0.021 鹵0.024) and Pax7 (0.014 鹵0.004) in children with exotropia were not significantly different from those in adult exotropia (P0.05). Conclusion: the weak lateral extraocular muscle fibers of the concomitant exotropia may be related to the occurrence and development of concomitant exotropia with atrophy, decreased morphologic changes, and decreased expression of MyoD, Pax7, which may be related to the occurrence and development of concomitant exotropia.
