【摘要】：目的觀察Artisan虹膜夾持型人工晶狀體(IOL)治療無晶狀體眼的臨床療效及手術(shù)并發(fā)癥。方法選取接受Artisan虹膜夾持型IOL植入術(shù)的無晶狀體眼患者13例(13眼),比較患者手術(shù)前后裸眼視力、最佳矯正視力和眼壓變化,裂隙燈顯微鏡觀察術(shù)后角膜水腫、前房閃輝、IOL位置和虹膜情況并行角膜內(nèi)皮細(xì)胞計數(shù)。結(jié)果術(shù)后隨訪平均2年,裸眼視力及最佳矯正視力超過0.5的有5只眼(38.5%)和6只眼(46.2%),手術(shù)前后眼壓無明顯變化,術(shù)后隨訪角膜無明顯水腫,晶狀體位置未見明顯偏移,前房無明顯炎癥反應(yīng),平均內(nèi)皮細(xì)胞丟失率為10.1%。術(shù)中有2例合并虹膜損傷穿孔,有2例發(fā)生虹膜出血,但均在術(shù)后短期內(nèi)吸收。術(shù)后隨訪1例IOL脫位,予重新手術(shù)固定。隨訪未發(fā)現(xiàn)黃斑囊樣水腫。結(jié)論 Artisan虹膜夾持型IOL植入術(shù)是一種安全有效的治療無晶狀體眼的方法,手術(shù)操作相對簡單,組織損傷相對較小,并發(fā)癥少,術(shù)后視力提高較為顯著。
[Abstract]:Objective to observe the clinical effect and operative complications of Artisan iridoid intraocular lens (IOL) in the treatment of aphakic eyes. Methods Thirteen patients (13 eyes) with aphakic eye underwent Artisan iridoplasty with IOL implantation were selected. The uncorrected visual acuity, best corrected visual acuity and intraocular pressure were compared before and after operation. Corneal edema was observed by slit lamp microscope. The position of IOL and the iris of anterior chamber were counted with corneal endothelial cells. Results the mean postoperative follow-up was 2 years. 5 eyes with uncorrected visual acuity and best corrected visual acuity (BCVA) (38.5%) and 6 eyes (46.2%) had no obvious change in IOP before and after operation. There was no obvious edema in cornea and no obvious deviation of lens position. There was no obvious inflammatory reaction in anterior chamber, and the average loss rate of endothelial cells was 10.1%. There were 2 cases with iris injury and perforation and 2 cases with iris hemorrhage during the operation, but all of them were absorbed in a short period after operation. One case of IOL dislocation was followed up and fixed with reoperation. No macular cystic edema was found at the follow-up. Conclusion Artisan iridoplasty with IOL implantation is a safe and effective method for the treatment of aphakic eyes. The operation is relatively simple, the tissue injury is relatively small, the complications are less, and the postoperative visual acuity is improved significantly.
