【摘要】:阻塞性睡眠呼吸暫停綜合征(OSAS)是一種發(fā)病率較高且具有一定潛在威脅的疾病。醫(yī)學(xué)上,多導(dǎo)睡眠儀(PSG)是公認(rèn)的檢測OSAS的黃金標(biāo)準(zhǔn),已被廣泛地應(yīng)用到OSAS檢測中,但其檢測費(fèi)用昂貴,對檢測環(huán)境要求較高等不足極大地制約了PSG的普及化使用。完全依靠PSG對眾多OSAS潛在人群進(jìn)行逐一排查難度非常大,據(jù)不完全統(tǒng)計,有80%-90%的患者得不到及時診治。因此,找到一種對患者睡眠干擾小,成本低,易于操作,適用于大量人群檢測的篩查方法成為亟待解決的問題。 OSAS又稱為“鼾癥”,鼾聲是OSAS最直接的臨床表征,其中包含了反應(yīng)上氣道異常的足夠信息。此外,鼾聲的獲取簡便快捷;谏鲜鲈,利用鼾聲特性實現(xiàn)OSAS檢測成為本領(lǐng)域研究的熱點(diǎn)。本文從實際角度出發(fā),對有環(huán)境噪聲影響下的魯棒性O(shè)SAS檢測方法問題進(jìn)行研究,系統(tǒng)的分析了家庭環(huán)境中基于鼾聲特性的OSAS檢測方法并對不同患病等級的OSAS患者進(jìn)行相應(yīng)研究。本文的研究內(nèi)容主要有以下幾個方面: 1)提出子帶能量比篩查方法。該方法可作為一種類似于常規(guī)體檢的檢測方法,對眾多潛在OSAS人群進(jìn)行篩查,然后將篩查呈陽性的患者(OSAS高危者)交由PSG進(jìn)一步檢查確診,從而大大減小PSG檢測的負(fù)荷。 2)因為患者患病程度越高,則上氣道阻塞程度越嚴(yán)重,其頻譜能量集中頻段越高。本文利用子帶能量比的思想進(jìn)一步對不同患病等級的OSAS患者進(jìn)行研究。 3)既然將本文方法定位為OSAS的篩查方法,方法的抗噪性能是一個至關(guān)重要的因素,本文驗證了此方法在強(qiáng)噪聲干擾情況下的篩查能力。 實驗結(jié)果表明本方法能較好的克服背景噪聲對檢測的影響,在家庭環(huán)境中亦能達(dá)到較高的檢測精度,其靈敏度為92.8%,特異性為92.7%,已基本滿足OSAS篩查的實際應(yīng)用要求。此外對不同患病等級患者的研究今后利用鼾聲特性區(qū)分不同患病程度提供一種有效的研究思路。
[Abstract]:Obstructive sleep apnea syndrome (OSAS) is a disease with high incidence and potential threat. In medicine, polysomnography (PSG) is recognized as the gold standard for the detection of OSAS, which has been widely used in OSAS detection. However, its high detection cost and high environmental requirements have greatly restricted the widespread use of PSG. It is very difficult to investigate the potential population of OSAS by PSG completely. According to incomplete statistics, 80% to 90% of the patients can not be diagnosed and treated in time. Therefore, it is an urgent problem to find a screening method for patients with low sleep disturbance, low cost, easy to operate and suitable for mass detection. OSAS, also known as snoring, is the most direct clinical manifestation of OSAS, which contains enough information to respond to upper airway abnormalities. In addition, snore acquisition is convenient and fast. For the above reasons, using snoring characteristics to realize OSAS detection has become a hotspot in this field. In this paper, the problem of robust OSAS detection method under the influence of environmental noise is studied from the practical point of view. The OSAS detection method based on snoring characteristics in the home environment is systematically analyzed and the corresponding research on OSAS patients with different disease grades is carried out. The main contents of this paper are as follows: 1) the subband energy ratio screening method is proposed. This method can be used as a kind of detection method similar to the routine physical examination to screen many potential OSAS population, and then to submit the screening positive patients to PSG for further diagnosis, thus greatly reducing the load of PSG detection. 2) the higher the degree of disease, the more severe the obstruction of upper airway and the higher the frequency band of concentrated spectrum energy. In this paper, subband energy ratio is used to further study OSAS patients with different disease grades. 3) since this method is positioned as the screening method of OSAS, the anti-noise performance of the method is a crucial factor, and this paper verifies the screening ability of this method in the case of strong noise interference. The experimental results show that this method can overcome the influence of background noise on the detection and achieve a high detection accuracy in the home environment. The sensitivity is 92.8 and the specificity is 92.70.This method has basically met the practical application requirements of OSAS screening. In addition, the study of patients with different disease grades provides an effective way to distinguish different degrees of disease by using snoring characteristics in the future.
