【摘要】：目的:探討影響幼年型復發(fā)性呼吸道乳頭狀瘤臨床病程和嚴重程度的因素。方法:回顧分析幼年型復發(fā)性呼吸道乳頭狀瘤患兒的臨床資料,分別按發(fā)病年齡、首次復發(fā)年齡、首次復發(fā)間期、性別、有無氣管切開進行分組,并對病變嚴重程度評分與受累的亞區(qū)、手術(shù)間期以及再次手術(shù)的間期進行相關(guān)性分析。結(jié)果:按復發(fā)年齡分為≤4歲組和4歲組,2組在手術(shù)次數(shù)和平均、最短、最長手術(shù)間期差異有統(tǒng)計學意義(P值分別為0.029,0.003,0.010,0.039),而按發(fā)病年齡分組(≤4歲組和4歲組)則差異無統(tǒng)計學意義(P值分別為0.154,0.219,0.194,0.219);病灶嚴重程度評分與受累的亞區(qū)呈正相關(guān)(r=0.914,P=0.000),與復發(fā)間期呈負相關(guān)(r=-0.451,P=0.000);發(fā)病年齡≤4歲組和4歲組在病灶嚴重程度和手術(shù)間期上差異均有統(tǒng)計學意義(P值分別為0.036,0.000),而按手術(shù)年齡分組則無明顯差異(P值分別為0.103,0.813);行氣管切開術(shù)的患兒接受更多的手術(shù),但在發(fā)病年齡、復發(fā)年齡及復發(fā)間期等上未見明顯差異。結(jié)論:準確評估幼年型復發(fā)性呼吸道乳頭狀瘤臨床病程的發(fā)展趨勢,首先要考慮首次復發(fā)年齡及間期,判斷病灶嚴重程度需考慮發(fā)病年齡及病變受累的亞區(qū)。
