高透氧夜戴式角膜塑型片對(duì)角膜內(nèi)皮細(xì)胞的影響

【摘要】:研究背景現(xiàn)代近視普遍率越來越高,度數(shù)也越來越朝高度數(shù)發(fā)展。在社會(huì)競(jìng)爭(zhēng)的壓力下,學(xué)齡前的提早學(xué)習(xí),不管是術(shù)科或?qū)W科都趨之若鶩。父母親為了一句”不要輸在起跑點(diǎn)”,一窩蜂式的去補(bǔ)習(xí)、去學(xué)特殊技能。眼睛在提早過度使用之下,近視也提早出視。視力不良會(huì)影響到日常生活,使上課學(xué)習(xí)能力下降。矯正方式最為普遍的就是戴一般的光學(xué)眼鏡,其次還有隱形眼鏡、屈光手術(shù)治療、角膜塑型治療。 各種視力矯正方式都有它的優(yōu)缺點(diǎn),不同年齡層、不同工作性質(zhì)各有所需求。以學(xué)童來說除了方便還要考慮安全性,光學(xué)眼鏡的方便性高,戴在活力十足的學(xué)童臉上就有些不便。近年來為了延遲兒童近視快速增加并且矯正視力,夜戴式角膜塑型術(shù)日漸普遍。日戴式高透氧隱形眼鏡長時(shí)間配戴之下,多少都會(huì)造成角膜缺氧。而夜戴式角膜塑型片是否也一樣讓角膜缺氧或有其它變化,值得進(jìn)一步探討。 目的 本實(shí)驗(yàn)使用角膜影像分析儀,分析學(xué)童配戴夜戴式角膜塑型片前和配戴兩年后其的,依據(jù)其六角形細(xì)胞及細(xì)胞密度的變化來推論角膜在配戴夜戴式角膜塑型片后所受的影響。 方法1研究人群:選取7歲到15歲的學(xué)童,14名男性,20名女性,無眼部疾病。 2近視度數(shù)-1.0D到-5.0D,散光0.00D到-1.75D3配戴夜戴式角膜塑型片前以MONMET角膜曲度計(jì)測(cè)量左右眼角膜曲度,TOPCON SP-3000P角膜影像分析儀測(cè)量其角膜內(nèi)皮的六角形細(xì)胞出現(xiàn)頻率及細(xì)胞密度,二年后再同樣一次。 4第一次配戴夜戴式角膜塑型片后三天首次追蹤,如果正常,7天后第二次追蹤檢查,14天后第三次檢查,1個(gè)月后第四次檢查,如果無異常,日后每3個(gè)月檢查一次。 結(jié)果1受測(cè)者34名都正常配戴,其中5名在這2年內(nèi)曾經(jīng)因?yàn)檫^敏性結(jié)膜炎而暫時(shí)停止配戴2天,愈后正常配戴。 2受測(cè)者左右眼配戴夜戴式角膜塑型片前、后2年的角膜內(nèi)皮細(xì)胞密度無統(tǒng)計(jì)學(xué)上的差異(p=0.230paired t test)。 3治療前角膜內(nèi)皮六角形細(xì)胞出現(xiàn)頻率與治療后2年角膜內(nèi)皮六角形細(xì)胞出現(xiàn)頻率無統(tǒng)計(jì)學(xué)上的差異(p=0.131paired t test)。 結(jié)論 1.配戴夜戴式角膜塑型可以暫時(shí)達(dá)到減少近視度數(shù)的效果。 2配戴夜戴式角膜塑型2年內(nèi)對(duì)角膜內(nèi)皮細(xì)胞的形態(tài)與細(xì)胞密度無明顯的影響。 3長期配戴夜戴式角膜塑型對(duì)角膜的影響仍然須要進(jìn)一步的探討。
[Abstract]:Background the prevalence and degree of modern myopia are increasing. Under the pressure of social competition, preschool early learning, be it technical or academic. For the sake of saying "Don't lose at the starting point", parents swarm to study and learn special skills. Eyes under early overuse, myopia also early out of sight. Poor eyesight can affect daily life and reduce the ability to learn in class. The most common correction is to wear general optical glasses, followed by contact lenses, refractive surgery, corneal plastic treatment. All kinds of visual acuity correction methods have their own advantages and disadvantages, different age groups, different working properties have their own needs. In addition to convenience and safety considerations, optical glasses are highly convenient and inconvenient to wear on the faces of energetic schoolchildren. In recent years, in order to delay the rapid increase of myopia and correct vision in children, night-wear keratoplasty is becoming more and more common. Wearing high oxygen-permeable contact lenses for a long time can cause corneal anoxia. It is worth further study whether the night-wear keratoplasty also causes anoxia or other changes in the cornea. Purpose In this experiment, the corneal image analyzer was used to analyze the influence of the children on the cornea after wearing the night-wear keratoplasty and two years later. According to the changes of the hexagonal cells and cell density, the influence of the cornea after wearing the night-wear corneal plastic film was deduced. Methods 1 A total of 14 male and 20 female children aged 7 to 15 years were enrolled in the study. (2) myopia degree -1.0D to -5.0D, astigmatism 0.00D to -1.75D3 was used to measure the frequency and cell density of hexagonal cells in corneal endothelium by MONMET keratometer before wearing corneal plastic film. Two years later again. (4) for the first time, 3 days after wearing the night-wear keratoplasty, if normal 7 days later, the second follow-up examination was followed by the third one after 14 days, the fourth time after one month, and if there was no abnormality, every three months in the future. Results (1) 34 subjects were normally worn, 5 of them had been temporarily stopped wearing for 2 days because of allergic conjunctivitis. 2There was no significant difference in the density of corneal endothelial cells between the left and right eyes of the subjects after 2 years before wearing night keratoplasty, and there was no significant difference in the density of corneal endothelial cells between the two groups (P < 0. 230 paired t test). 3There was no significant difference between the frequency of corneal endothelial hexagonal cells before treatment and that of corneal endothelial hexagonal cells 2 years after treatment. Conclusion 1. Wearing night-wearing keratoplasty can temporarily reduce myopia. 2 the morphology and cell density of corneal endothelial cells were not significantly affected by wearing night-wear keratoplasty for 2 years. 3 the effect of long-term wearing night-wear keratoplasty on cornea still needs further discussion.
