【摘要】： 目的通過多種方式、多角度充分顯露顳骨相關解剖結構及毗鄰關系,將微細的結構直觀化展現(xiàn),給臨床工作者以整體概念,再結合手術角度確定標志,為臨床教學及耳科疾病的影像診斷、手術定位提供參考。 方法由鄭州衛(wèi)教實驗用品研制中心提供排除耳部器質性疾病的成年男性耳顳部材料(干性顳骨5對,濕性顳骨10對)、器械試劑、技術指導及場地等,通過磨制雕琢、內耳灌注、冰凍鋸切等多種方法,制作顳骨解剖標本,充分顯露相關結構及毗鄰關系；使用尼康數碼單鏡反光相機D80及AF-S微距尼克爾60mm f／2.8G ED鏡頭多角度多參數采集圖片；運用photoshop等軟件處理標注圖片。 結果1.獲得大量顳骨解剖標本及實物圖片：(1)顯示顳骨基本解剖結構及定位標志；(2)通過斷層、內耳鑄型、前庭蝸器磨制雕琢等多種方式,多角度充分顯示內耳道、鼓室、耳蝸、前庭、半規(guī)管等精細復雜結構的位置及毗鄰；(3)顯示顳骨內面神經全程走行、分支及毗鄰；(4)參照手術徑路顯示解剖標志及毗鄰關系。 2.結合《耳鼻咽喉-頭頸應用解剖學》相關章節(jié)內容,修改配圖,明確各解剖標志的手術定位意義,制作直觀性強的教學圖庫。 結論實物標本結合圖像資料,多角度充分展示耳顳部微細解剖關系,結構清晰、形象直觀；幫助臨床工作者建立顳骨精細結構的三維立體空間概念,在此基礎上結合手術徑路,明確解剖標志的定位意義,融匯貫通,提高耳鼻咽喉科的教學效果,指導影像診斷及術中定位,降低耳神經外科手術風險,服務臨床。
[Abstract]:Objective to expose the temporal bone related anatomical structure and its adjacent relationship in various ways and to visualize the fine structure, and to give the clinical workers the whole concept and the operative angle to determine the mark.To provide reference for clinical teaching, imaging diagnosis and surgical location of otological diseases.Methods the materials of adult male ear-temporal region (5 pairs of dry temporal bone, 10 pairs of wet temporal bone, instrument reagent, technical guidance and site, etc.) were provided by Zhengzhou Health Education Experimental equipment Development Center.Frozen sawing and other methods were used to make anatomical specimens of temporal bone to fully reveal the related structures and adjacent relationships, and to collect images by using the Nikon digital single mirror reflex camera D80 and AF-S microscoped 60mm f / 2.8 G Ed lens.Use photoshop and other software to process tagged pictures.Result 1.A large number of anatomical specimens and images of temporal bones were obtained, showing that the basic anatomical structure of the temporal bone and its localization mark were as follows: through a variety of methods, such as faults, inner ear molds, vestibule cochlea grinding and carving, the inner ear canal, tympanic chamber, cochlea and vestibule were fully displayed at various angles.The location and adjacent structure of semicircular canal and intra facial nerve were demonstrated in the whole course of the temporal bone, and the branches and adjacent fir 4) showed the anatomic signs and the adjacent relationship according to the operative path.2.Combined with the relevant chapters of applied anatomy of ear, nose, throat and head and neck, the matching map was revised, the significance of surgical positioning of each anatomic mark was clarified, and a teaching map library with strong visual sense was made.Conclusion the object specimen combined with the image data can fully display the fine anatomical relationship of the temporal part of the ear in many angles, the structure is clear and the image is intuitionistic, the three-dimensional spatial concept of the fine structure of the temporal bone is established by the clinical workers, and the operative approach is combined with the concept of the three-dimensional space of the fine structure of temporal bone.The purpose of this study was to clarify the localization significance of anatomic markers, to improve the teaching effect of otorhinolaryngology, to guide imaging diagnosis and intraoperative positioning, to reduce the risk of otoneurosurgery, and to serve clinical practice.
