【摘要】：目的：探討E-鈣粘附蛋白在28例視網(wǎng)膜母細胞瘤腫瘤組織中的表達情況及與臨床病理特征之間的相關性。 方法：應用免疫組織鏈霉菌抗生物素蛋白-過氧化物酶連結化學染色法(streptavidin-perosidase, SP法)檢測28例視網(wǎng)膜母細胞瘤組織及7例正常視網(wǎng)膜組織中E-鈣粘附蛋白的表達；利用SPSS13.0統(tǒng)計分析軟件處理,并比較E-鈣粘附蛋白在正常視網(wǎng)膜組織和RB組織中的表達,分析E-鈣粘附蛋白表達與RB病例臨床病理特征的聯(lián)系以及RB組織中E-鈣粘附蛋白表達分級與腫瘤分期、病理分級、是否侵犯視神經(jīng)和預后的相關性。 結果： 1 E-鈣粘附蛋白在視網(wǎng)膜母細胞瘤中的弱陽性表達率42.8%(12/28)高于其在正常視網(wǎng)膜組織的弱陽性表達率0%(0/7)；.E-鈣粘附蛋白在視網(wǎng)膜母細胞瘤中的強陽性表達率35.7%(10/28)低于其在正常視網(wǎng)膜組織的強陽性表達率85.7%(6/7)。 2按照視網(wǎng)膜細胞瘤病理分型(低分化型和高分化型),腫瘤分期(眼內期和眼外期),復發(fā)與初發(fā),是否視神經(jīng)浸潤分組,E-鈣粘附蛋白的弱陽性表達率的在兩組之間差異(低分化型高于高分化型,眼內期低于眼外期,復發(fā)組高于初發(fā)組,有視神經(jīng)浸潤高于無視神經(jīng)浸潤組)均有統(tǒng)計學意義(P0.05),與患者年齡、性別、預后無關(P0.05)。 3 E-鈣粘附蛋白陽性表達程度與視網(wǎng)膜母細胞瘤的腫瘤分期呈負相關(r=-0.472 p0.01),與視神經(jīng)的浸潤和轉移,病理分級呈正相關(r=0.472p0.01,r=0.509p0.01),與預后無明顯相關性(r=-0.312P0.01)。 結論：E-鈣粘附蛋白的表達下降與視網(wǎng)膜母細胞瘤視神經(jīng)浸潤,腫瘤的轉移,分化,腫瘤的分型,腫瘤的復發(fā)有密切的關系,所以檢測E-鈣粘附蛋白在視網(wǎng)膜母細胞瘤組織中的表達對判斷及預測視網(wǎng)膜母細胞瘤的惡性程度與轉移有一定的意義。
[Abstract]:Objective: to investigate the expression of E-cadherin in 28 cases of retinoblastoma and its correlation with clinicopathological features. Methods: immunohistochemical staining of streptavidin-perosidase (SP) was used to detect the expression of E-cadherin in 28 cases of retinoblastoma and 7 cases of normal retina. The expression of E-cadherin in normal retina and RB was compared by SPSS13.0 software. To analyze the relationship between the expression of E-cadherin and the clinicopathological features of RB, and the correlation between the expression of E-cadherin and tumor stage, pathological grade, optic nerve invasion and prognosis. Results:. (1) the weak positive rate of E-cadherin in retinoblastoma was 42.812 / 28) higher than that in normal retinal tissue. The strong positive rate of E-cadherin in retinoblastoma was 35.710 / 28) lower than that in normal retinal tissue. The strong positive expression rate of normal retina was 85.7%. 2 according to the pathological classification of retinoma (low differentiation type and high differentiation type, tumor stage (intraocular phase and extraocular stage), recurrence and initial stage, the difference between the two types was observed. The difference of weak positive expression rate of E-cadherin between the two groups (low differentiation type is higher than high differentiation type, intraocular phase is lower than extraocular phase, recurrence group is higher than primary group. The optic nerve infiltration was higher in the group with optic nerve invasion than that in the group without optic nerve invasion (P 0.05), and had no correlation with age, sex and prognosis (P 0.05). 3 the positive expression of E-cadherin was negatively correlated with the tumor stage of retinoblastoma, and was positively correlated with the invasion and metastasis of optic nerve, and the pathological grade was positively correlated with the prognosis of retinoblastoma. There was no significant correlation between the expression of E-cadherin and the prognosis of retinoblastoma. Conclusion the decrease of the expression of E-cadherin in retinoblastoma is closely related to optic nerve invasion, metastasis, differentiation, classification and recurrence of retinoblastoma. Therefore, the expression of E-cadherin in retinoblastoma is of significance in judging and predicting the malignancy and metastasis of retinoblastoma.
