【摘要】： 目的:探討磁共振成像對(duì)鼻咽癌放射治療后改變的影像診斷價(jià)值,尤其是放療后復(fù)發(fā)的診斷及鑒別診斷。方法:收集新疆腫瘤醫(yī)院2006年1月-2009年10月經(jīng)病理證實(shí)的鼻咽癌病例67例,其中39例行彌散加權(quán)像(DWI)掃描。所有病例均行放射治療、有完整MR復(fù)查資料,分析其放療后不同情況下的MRI表現(xiàn),及MRI對(duì)復(fù)發(fā)或殘留的檢出,并進(jìn)一步利用彌散加權(quán)像測(cè)定的ADC值來(lái)研究放療后復(fù)發(fā)與纖維化及殘留的鑒別。結(jié)果:①M(fèi)RI基本能反應(yīng)放療后咽壁不同變化情況;②MR最初對(duì)復(fù)發(fā)或殘留的檢出,敏感性85.00%,特異性91.49%,準(zhǔn)確性89.55%;③放療后鼻咽纖維化者ADC值較復(fù)發(fā)者及殘留者明顯高(P0.05),而復(fù)發(fā)者咽后淋巴結(jié)ADC值較纖維化者及殘留者明顯低(P0.05)。結(jié)論:MRI能清晰地顯示鼻咽癌放療后變化情況,并且對(duì)復(fù)發(fā)的檢出準(zhǔn)確性較高,放療后纖維化、殘留及復(fù)發(fā)者ADC值的不同或許為鑒別診斷提供一定參考價(jià)值,MRI對(duì)放射治療鼻咽癌的效果評(píng)價(jià)至關(guān)重要。
[Abstract]:Objective: to evaluate the diagnostic value of magnetic resonance imaging (MRI) in patients with nasopharyngeal carcinoma (NPC) after radiotherapy. Methods: 67 cases of nasopharyngeal carcinoma (NPC) confirmed by pathology from January 2006 to 2009 in Xinjiang tumor Hospital were collected, 39 of them underwent DWI scan. All cases were treated with radiotherapy. There were complete Mr reexamination data, the MRI manifestations of the patients under different conditions after radiotherapy, and the detection of recurrence or residual by MRI. The differential diagnosis of recurrence and fibrosis and residual after radiotherapy was studied by using the ADC value measured by diffusion weighted imaging. Results the differential diagnosis of recurrence and residual in pharynx wall after radiotherapy was basically reflected by different changes in pharyngeal wall after radiotherapy. The sensitivity was 85.00 and the specificity was 91.49. The accuracy of ADC in patients with nasopharyngeal fibrosis after radiotherapy was significantly higher than that in patients with recurrent nasopharynx fibrosis and those with residual nasopharynx (P 0.05), while the ADC value of retropharyngeal lymph nodes in recurrent patients was significantly lower than that in patients with fibrosis and residual nasopharynx (P 0.05). Conclusion\% ADC can clearly show nasopharynx fibrosis. Changes in cancer after radiotherapy, The difference of ADC values of fibrosis, residual and recurrence after radiotherapy may provide a certain reference value for differential diagnosis. It is very important to evaluate the effect of radiotherapy on nasopharyngeal carcinoma.
