【摘要】：目的:分析額隱窩氣房與慢性額竇炎的相關(guān)性。方法:收集69例(慢性額竇炎組37例,對(duì)照組32例)符合病例選擇標(biāo)準(zhǔn)的鼻竇CT影像,在圖像工作站上進(jìn)行軸位、冠狀位、矢狀位重建獲得額隱窩區(qū)域影像(層厚0.6mm,層距0.4mm)。根據(jù)Lee等的額隱窩氣房CT分類判定標(biāo)準(zhǔn),應(yīng)用圖像分析軟件,觀察慢性額竇炎組與對(duì)照組額隱窩氣房的分布類型,比較兩組的差異,應(yīng)用Logistic回歸分析額隱窩氣房與慢性額竇炎的相關(guān)性。結(jié)果:獲得128側(cè)額隱窩區(qū)域三維影像,每組各64側(cè),其中Ⅲ型額氣房、眶上篩房在慢性額竇炎組的出現(xiàn)率(分別為37.5%和70.3%)顯著高于對(duì)照組(分別為12.5%和48.4%),差異均有統(tǒng)計(jì)學(xué)意義(均P0.01);其他類型氣房在兩組中的出現(xiàn)率差異無統(tǒng)計(jì)學(xué)意義。Logistic回歸分析顯示Ⅲ型額氣房(OR=3.740,P=0.005)、眶上篩房(OR=2.242,P=0.031)與慢性額竇炎相關(guān)。結(jié)論:Ⅲ型額氣房、眶上篩房可能是導(dǎo)致額竇引流通道狹窄的關(guān)鍵解剖因素,與慢性額竇炎密切相關(guān)。
[Abstract]:Objective: to analyze the correlation between frontal recess gas chamber and chronic frontal sinusitis. Methods: Ct images of paranasal sinuses of 69 cases (37 cases of chronic frontal sinusitis group and 32 cases of control group) were collected, and axial and coronal images were performed on the image workstation. Sagittal reconstruction was performed to obtain frontal recess images (slice thickness 0.6 mm, interval 0.4 mm). According to CT classification criteria of frontal recess of Lee et al, the distribution types of frontal recess in chronic frontal sinusitis group and control group were observed by using image analysis software. The correlation between frontal recess gas chamber and chronic frontal sinusitis was analyzed by Logistic regression analysis. Results: three dimensional images of frontal recess region were obtained in each group, 64 sides in each group. The occurrences of supraorbital ethmoid chamber in chronic frontal sinusitis group (37.5% and 70.3 respectively) were significantly higher than those in control group (12.5% and 48.4, respectively) (P < 0.01), but there was no significant difference in other types of gas chamber in the two groups.
