角蛋白19表達(dá)與鼻咽癌臨床病理特征的關(guān)系研究

【摘要】:目的研究角蛋白19(Cytokeratin 19或Keratin 19, CK19)在鼻咽癌組織和細(xì)胞中的表達(dá)情況,分析其表達(dá)與鼻咽癌臨床病理特征的關(guān)系。 方法62例鼻咽癌、17例淋巴結(jié)轉(zhuǎn)移性鼻咽癌和16例慢性鼻咽炎(作為正常鼻咽粘膜對照組)標(biāo)本均由病理證實,采用免疫組化S-P法檢測其中CK19的表達(dá)水平,并分析其表達(dá)水平與鼻咽癌臨床病理特征的關(guān)系;采用western blot檢測不同分化程度和不同轉(zhuǎn)移潛能NPC細(xì)胞系中CK19蛋白的表達(dá)情況。 結(jié)果 1)免疫組化結(jié)果顯示CK19主要表達(dá)在細(xì)胞胞漿和胞膜,鼻咽癌CK19陽性率為74.2%(46/62);淋巴結(jié)轉(zhuǎn)移性鼻咽癌中陽性率為88.2%(15/17);對照組正常鼻咽上皮中陽性率為12.5%(2/16);CK19表達(dá)在淋巴結(jié)轉(zhuǎn)移性鼻咽癌高于鼻咽癌標(biāo)本,在鼻咽癌中高于正常對照組,差異均有顯著性意義(P0.05) 2)CK19表達(dá)與NPC分化有關(guān):在中分化鼻咽癌中CK19陽性率為46.2%(6/13),低分化鼻咽中陽性率為81.6%(40/49),兩者之間差異有顯著性意義(P0.05)。 3)CK19表達(dá)與NPC原發(fā)瘤大小有關(guān):CK19表達(dá)在無侵犯組的陽性率為20.0%(1/5),而在有周圍侵犯組中的陽性率為78.9%(45/57),兩者之間有顯著性差異(P0.05)。 4)CK19表達(dá)與NPC淋巴結(jié)轉(zhuǎn)移有關(guān):CK19表達(dá)在無淋巴結(jié)轉(zhuǎn)移的NPC中陽性率為11.1%(1/9),而在有淋巴結(jié)轉(zhuǎn)移的NPC中為84.9%(45/53),兩者之間有顯著性差異。 5)CK19表達(dá)與NPC遠(yuǎn)處結(jié)轉(zhuǎn)移有關(guān):CK19在無遠(yuǎn)處轉(zhuǎn)移的NPC中陽性率為71.4%(40/56),在有遠(yuǎn)處轉(zhuǎn)移的NPC中為100%(6/6),兩者之間有顯著性差異(P0.05)。 6)CK19蛋白表達(dá)與NPC臨床分期有關(guān):Ⅰ+Ⅱ期NPC組織中CK19表達(dá)的陽性率為45.5%(5/11),而在Ⅲ+Ⅳ期的NPC中CK19表達(dá)的陽性率為78.8%(41/52),兩者之間有顯著性差異(P0.05)。 7)CK19蛋白表達(dá)與性別、年齡無關(guān)(P0.05) 8) Western blot結(jié)果顯示,CK19在低分化NPC細(xì)胞系CNE2中的表達(dá)水平較高分化NPC細(xì)胞系CNE1明顯上調(diào);在高轉(zhuǎn)移NPC細(xì)胞系5-8F中的表達(dá)水平較低轉(zhuǎn)移NPC細(xì)胞系6-10B明顯上調(diào)(P0.01),說明CK19表達(dá)與NPC分化及轉(zhuǎn)移有關(guān)。 結(jié)論 1)鼻咽癌組織中CK19表達(dá)在淋巴結(jié)轉(zhuǎn)移性鼻咽癌高于原發(fā)灶,在鼻咽癌中高于慢性鼻咽炎;分化差和高轉(zhuǎn)移潛能的的鼻咽癌細(xì)胞中CK19表達(dá)高于分化好和低轉(zhuǎn)移潛能的鼻咽癌細(xì)胞株,CK19可作為鼻咽癌分化和轉(zhuǎn)移的分子標(biāo)記物。 2)CK19表達(dá)與原發(fā)瘤大小、分化程度、淋巴結(jié)轉(zhuǎn)移和遠(yuǎn)處轉(zhuǎn)移、臨床分期有關(guān)。與年齡、性別無關(guān)。
[Abstract]:Objective to study the expression of keratin 19 or Keratin 19 (CK19) in nasopharyngeal carcinoma (NPC) tissues and cells, and to analyze the relationship between the expression and clinicopathological features of nasopharyngeal carcinoma (NPC). Methods Sixty-two cases of nasopharyngeal carcinoma with lymph node metastasis and 16 cases of chronic nasopharyngitis (as normal nasopharyngeal mucosa control group) were confirmed by pathology. Immunohistochemical S-P method was used to detect the expression of CK19. The relationship between the expression level and clinicopathological features of nasopharyngeal carcinoma was analyzed, and the expression of CK19 protein in NPC cell lines with different differentiation and metastasis potential was detected by western blot. Results. 1) Immunohistochemical results showed that CK19 was mainly expressed in cytoplasm and membrane. The positive rate of CK19 in nasopharyngeal carcinoma was 74.2 / 66 / 62; the positive rate of lymph node metastatic nasopharyngeal carcinoma was 88.2 / 15 / 17; the positive rate of CK19 in normal nasopharyngeal epithelium in control group was 12.5% and the positive rate of CK19 in lymph node metastatic nasopharyngeal carcinoma was higher than that in nasopharyngeal carcinoma specimen, and was higher in nasopharyngeal carcinoma than in normal control group. The difference was significant (P0.05). 2the expression of CK19 was related to the differentiation of NPC: the positive rate of CK19 in moderately differentiated nasopharyngeal carcinoma was 46.2 / 13, and the positive rate in poorly differentiated nasopharynx was 81.6 / 49. There was a significant difference between the two groups (P 0.05). 3The expression of NPC CK19 was related to the size of the primary tumor of NPC. The positive rate of the expression of NPC in the non-invasive group was 20.0 / 5%, while in the group with peripheral invasion, the positive rate was 78.9% / 57%. There was a significant difference between the two groups (P 0.05). (4) the positive rate of NPC CK19 expression in NPC without lymph node metastasis was 11. 1 / 9 / 9, and that in NPC with lymph node metastasis was 84. 9 / 53. There was a significant difference between the two groups. 5The expression of CK19 was related to the distant node metastasis of NPC.
