【摘要】：目的:根據(jù)美國心血管造影及介入?yún)f(xié)會(SCAI)新標準評估經(jīng)皮冠狀動脈介入治療(PCI)術(shù)后圍術(shù)期心肌梗死(PMI)的相關危險因素。方法:根據(jù)最新的SCAI標準選取我院PCI術(shù)后PMI的患者3 371例(3 516例次擇期PCI),回顧性分析所有患者的臨床基線特征、造影結(jié)果以及操作特點,應用多因素回歸分析識別PMI的獨立危險因素。結(jié)果:入選患者共發(fā)生108次(3.1%)PMI。經(jīng)多因素回歸分析,年齡[比值比(OR)=1.037,95%可信區(qū)間(CI):1.016~1.058]、處理多支血管病變(OR=1.697,95%CI:1.095~2.629)、至少處理一處分叉病變(OR=1.869,95%CI:1.213~2.878)以及靶病變總長度(OR=1.016,95%CI:1.009~1.024)是PMI的獨立危險因素。結(jié)論:年齡、多支病變PCI、至少處理一處分叉病變以及靶病變總長度是發(fā)生PMI的獨立危險因素。
[Abstract]:Aim: to evaluate the risk factors of perioperative myocardial infarction (PMI) after percutaneous coronary intervention (PCI) according to the new criteria of American Cardiovascular Angiography and Interventional Association (SCAI). Methods: according to the latest SCAI criteria, 3 371 patients with PMI after PCI in our hospital were selected. (3 516 cases of selective PCI),) were retrospectively analyzed the clinical baseline characteristics, angiographic results and operation characteristics of all patients. Multivariate regression analysis was used to identify independent risk factors of PMI. Results: PMI. occurred 108 times (3.1%) in the selected patients. Multivariate regression analysis showed that age [ratio (OR) = 1.037, 95% confidence interval (CI): 1.016 脳 1.058] was used to treat multiple vessel disease (OR=1.697,95%CI:1.095~2.629). At least one bifurcation lesion (OR=1.869,95%CI:1.213~2.878) and total target lesion length (OR=1.016,95%CI:1.009~1.024) were independent risk factors for PMI. Conclusion: age, PCI, treatment of at least one bifurcation lesion and total length of target lesion are independent risk factors for PMI.
