【摘要】：目前我國(guó)高血壓患病率及并發(fā)癥增長(zhǎng)迅速,已成為中國(guó)人群心腦血管疾病最重要的危險(xiǎn)因素之一。高血壓的防治僅依靠醫(yī)院門診是不夠的,必須重視并加強(qiáng)社區(qū)人群預(yù)防、篩查和管理,改革和建立能提供長(zhǎng)期穩(wěn)定社區(qū)人群的服務(wù)體系。全科醫(yī)學(xué)團(tuán)隊(duì)干預(yù)是一種新型的、有效的社區(qū)干預(yù)模式,其高血壓社區(qū)健康管理模式在提高知曉率、生活方式改善、血壓達(dá)標(biāo)率、靶器官保護(hù)等方面起著積極的作用。本文就全科醫(yī)學(xué)團(tuán)隊(duì)式健康管理在高血壓患者中的應(yīng)用現(xiàn)狀進(jìn)行綜述。
[Abstract]:At present, the prevalence and complications of hypertension are increasing rapidly in China, which has become one of the most important risk factors for cardiovascular and cerebrovascular diseases in Chinese population. It is not enough to rely on hospital outpatient service for the prevention and treatment of hypertension. It is necessary to pay attention to and strengthen the prevention, screening and management of community population, and reform and establish a service system that can provide long-term stable community population. General practice team intervention is a new and effective community intervention model. Its hypertension community health management mode plays an active role in improving awareness rate, lifestyle improvement, blood pressure compliance rate, target organ protection and so on. This article reviews the application of group health management in patients with hypertension.
