踝—臂脈搏波傳導速度與冠心病的相關性研究

【摘要】:目的探討踝-臂脈搏波傳導速度(ba PWV)與冠心病的相關性方法選取2014年01月至2014年12月期間,在福建醫(yī)科大學附屬二院心內(nèi)科行冠狀動脈造影以及ba PWV檢查患者119例。根據(jù)冠狀動脈造影結果分為CHD組(56例)和對照組(63例),同時將CHD組進一步分為單支病變組(21例),雙支病變(17例),嚴重病變組(18例),比較冠心病各組與對照組的ba PWV水平,并對冠脈狹窄程度的評分(Gensini評分)與ba PWV水平作直線相關分析。以冠狀動脈造影為金標準,繪制ROC曲線評價ba PWV值對CHD的篩選價值。結果CHD組ba PWV顯著高于對照組(P0.05),進一步把冠心病組分成單支病變、雙支病變、嚴重病變組,所對應的ba PWV值分別為:1558.43±97.26cm/s、1698.00±100.49cm/s、1892.56±209.30cm/s,兩兩比較均有統(tǒng)計學意義(P0.05)。Ba PWV水平與Gensini積分作直線相關分析結果顯示呈正相關(r=0.886,P0.01)。由ROC曲線特征發(fā)現(xiàn),ba PWV的最佳截點為1525cm/s,此時Yuedden指數(shù)為0.728,敏感性83.9%,特異性88.9%。以ba PWV1525cm/s為ba PWV異常增高為因變量,做二項分類Logistic回歸分析,結果提示年齡(OR值1.112)、高血壓(OR值3.183)、糖尿病(OR值2.953)、HCY(OR值3.091)是ba PWV異常增高獨立危險因素。以冠心病有無為因變量,做二項分類Logistic回歸分析,結果顯示高血壓、糖尿病、ba PWV水平、吸煙、HCY是CHD獨立影響因素,其OR值分別為6.813、5.375、78.247、5.932、4.765。結論CHD與ba PWV等危險因素呈正相關關系。ba PWV對篩選CHD有一定的價值,并能夠預測冠脈病變的嚴重程度。
[Abstract]:Objective to investigate the correlation between ankle brachial pulse wave velocity (ba PWV) and coronary heart disease (CHD) methods 119 patients with coronary artery disease underwent coronary angiography and ba PWV examination from January 2014 to December 2014 in Department of Cardiology affiliated to Fujian Medical University. According to the results of coronary angiography, the patients were divided into CHD group (56 cases) and control group (63 cases). The CHD group was further divided into single vessel lesion group (21 cases), double vessel lesion group (17 cases) and severe lesion group (18 cases). The degree of coronary stenosis (Gensini score) and the level of ba PWV were analyzed by linear correlation. Using coronary angiography as gold standard, ROC curve was drawn to evaluate the screening value of ba PWV for CHD. Results ba PWV in CHD group was significantly higher than that in control group (P0.05). The corresponding ba PWV values were 1558.43 鹵97.26 cm / s, 1698.00 鹵100.49 cm / s and 1892.56 鹵209.30 cm / s, respectively. There was a positive correlation between the). Using ba PWV1525cm/s as dependent variable of abnormal increase of ba PWV, the results showed that age (OR value 1.112), hypertension (OR value 3.183) and diabetes mellitus (OR value 2.953), HCY (OR value 3.091) were independent risk factors for abnormal increase of ba PWV. The results showed that age (OR value 1.112), hypertension (OR value 3.183) and diabetes mellitus (OR 2.953), HCY (OR value 3.091) were independent risk factors for ba PWV abnormal increase. The results showed that hypertension, diabetes mellitus, ba PWV level, smoking and HCY were independent influencing factors of CHD, their OR values were 6.813 / 5.375 and 78.2475.932 / 4.765, respectively. Conclusion there is a positive correlation between CHD and ba PWV and other risk factors. Ba PWV has a certain value in screening CHD and can predict the severity of coronary artery disease.
