【摘要】：目的探討托伐普坦對(duì)慢性心力衰竭(CHF)患者容量負(fù)荷及血清鈉水平的影響。方法選取CHF患者94例,隨機(jī)分為對(duì)照組和觀察組各47例,兩組均給予常規(guī)CHF治療方案,觀察組另外給予托伐普坦,兩組療程均為3個(gè)月。兩組治療前后檢測(cè)心臟容量負(fù)荷指標(biāo),包括左室舒張末期壓力(LVEDP)、心輸出量(CO)、右心房壓(RAP)、肺動(dòng)脈嵌壓(PAWP)、肺動(dòng)脈壓(PAP);治療前及治療后1、3個(gè)月檢測(cè)血清鈉水平。結(jié)果兩組治療前LVEDP、CO、RAP、PAWP、PAP比較差異均無統(tǒng)計(jì)學(xué)意義(P均0.05);治療后兩組LVEDP、RAP、PAWP、PAP均較治療前降低,CO均較治療前升高,且觀察組上述變化更顯著(P均0.05)。治療前兩組血清鈉水平比較差異無統(tǒng)計(jì)學(xué)意義(P0.05);治療1、3個(gè)月時(shí),對(duì)照組血清鈉水平較治療前逐漸下降,觀察組血清鈉水平較治療前逐漸升高,且觀察組血清鈉水平均高于對(duì)照組,組間及組內(nèi)比較P均0.05。結(jié)論托伐普坦能有效降低CHF患者的容量負(fù)荷,并能預(yù)防低鈉血癥的發(fā)生。
[Abstract]:Objective to investigate the effect of torvalptan on volume load and serum sodium level in patients with chronic heart failure (CHF). Methods Ninety-four patients with CHF were randomly divided into two groups: the control group (n = 47) and the observation group (n = 47). Left ventricular end-diastolic pressure (LVEDP), cardiac output (CO), right atrial pressure (rap), pulmonary artery pressure (PAWP), pulmonary artery pressure (PAP), and serum sodium levels were measured before and after treatment. Results there was no significant difference between the two groups before and after treatment (P 0.05), and after treatment, the decrease of CO in the two groups was higher than that before treatment, and the above changes in the observation group were more significant (P 0.05). There was no significant difference in serum sodium levels between the two groups before treatment (P0.05); after 1 and 3 months of treatment, the serum sodium levels in the control group decreased gradually compared with those before treatment, and the serum sodium levels in the observation group gradually increased compared with those before treatment. The serum sodium level in the observation group was higher than that in the control group (P < 0.05). Conclusion Tofaptan can effectively reduce the volume load and prevent hyponatremia in CHF patients.
