【摘要】：目的探討分級診療背景下醫(yī)養(yǎng)結(jié)合健康管理模式對社區(qū)老年冠心病患者自我管理能力及生活質(zhì)量的影響。方法選取2013年3月—2015年5月在南方醫(yī)科大學南方醫(yī)院進行治療的80例老年冠心病患者為研究對象,采用隨機數(shù)字表法將其分為對照組和研究組,各40例。對照組給予常規(guī)健康管理方法,研究組在給予常規(guī)健康管理模式下,使用醫(yī)養(yǎng)結(jié)合健康管理模式進行干預。分別于干預前及干預半年后,對兩組患者自我管理能力以及生活質(zhì)量進行評分,檢測患者心功能主要指標左心室舒張末期內(nèi)徑(LVEDd)、每搏輸出量(SV)、射血分數(shù)(EF),并調(diào)查兩組患者干預后的滿意度。結(jié)果干預后,研究組急救管理、生活管理、飲食管理、服藥管理、不良嗜好管理、運動管理、情緒認知管理評分均高于對照組(P0.05);研究組生理功能、生理職能、軀體疼痛、總體健康、精力、社會職能、情感職能、精神健康評分均高于對照組(P0.05)。干預后,研究組LVEDd低于對照組,SV、EF高于對照組(P0.05)。兩組滿意度比較,差異有統(tǒng)計學意義(Z=6.23,P=0.01)。結(jié)論分級診療背景下醫(yī)養(yǎng)結(jié)合健康管理模式能有效提高社區(qū)老年冠心病患者的自我管理能力以及生活質(zhì)量,且患者滿意度高,值得推廣使用。
[Abstract]:Objective to explore the effect of medical and nursing combined with health management model on self-management ability and quality of life of elderly patients with coronary heart disease in community under the background of grading diagnosis and treatment.Methods from March 2013 to May 2015, 80 elderly patients with coronary heart disease were selected and divided into control group and study group with 40 cases each.The control group was given the routine health management method, the study group was given the routine health management mode, and the study group used the combination of medical care and health management mode to intervene.The self-management ability and quality of life of the two groups were evaluated before and six months after intervention.Results after intervention, the scores of emergency management, life management, diet management, medication management, bad hobby management, exercise management and emotional cognition management in the study group were higher than those in the control group (P 0.05), the physiological function, physiological function and somatic pain in the study group were higher than those in the control group.The scores of general health, energy, social function, emotional function and mental health were higher than that of control group (P 0.05).After intervention, the LVEDd of the study group was lower than that of the control group (P 0.05).There was significant difference in satisfaction between the two groups (P < 0.01).Conclusion in the background of graded diagnosis and treatment, the combination of medical and nursing combined with health management can effectively improve the self-management ability and quality of life of elderly patients with coronary heart disease in community, and the satisfaction of patients is high, which is worth popularizing.
