目的:探討肝硬化門(mén)靜脈血栓病人的血清脂質(zhì)譜在肝硬化嚴(yán)重程度上的差異以及早期預(yù)測(cè)預(yù)后的作用。方法:納入了肝硬化合并門(mén)靜脈血栓患者93例為研究對(duì)象。以2年預(yù)后分為非存活組16例,存活組77例。CTP A級(jí)18例,CTP B級(jí)55例,CTP C級(jí)20例。MELD＜10分46例,MELD≥10分47例。比較血脂等指標(biāo)在各組間的差異。進(jìn)一步行相關(guān)分析和生存分析。結(jié)果:1.非存活組和存活組之間在PLT、CHE、PT、PA、INR、HDL-C、CTP score、MELD score的比較有明顯差異。在年齡、性別、病因、脾切除術(shù)之間的比較無(wú)明顯差異。多因素分析示HDL-C和INR為預(yù)后的獨(dú)立影響因素。2.CHOL、HDL-C、LDL-C、Apo A在CTP三組之間比較差異有統(tǒng)計(jì)學(xué)意義,CTP A級(jí)明顯高于其他兩組。3.HDL-C、Apo A在MELD評(píng)分分組之間比較有明顯差異。4.CHOL、HDL-C、LDL-C、Apo A水平均與ALB、CHE水平存在正相關(guān),均與PT、INR、CTP、MELD評(píng)分存在負(fù)相關(guān)。5.HDL-C、CTP score、MELD score可預(yù)測(cè)肝硬化門(mén)靜脈血栓病人2年不良...
