【摘要】：目的評(píng)估瞬時(shí)彈性超聲成像儀(FibroTouch)與多參數(shù)模型在慢性肝病患者肝臟纖維化診斷中的臨床應(yīng)用價(jià)值及FibroTouch診斷肝纖維化分期的最佳診斷截點(diǎn)(cutoff)值。方法 2014年1月至2016年10月該院慢性肝病患者進(jìn)行肝臟組織活檢48例,收集血常規(guī)、肝功能等血清學(xué)指標(biāo),同時(shí)應(yīng)用FibroTouch進(jìn)行肝臟彈性測(cè)量值(LSM)測(cè)量,分析LSM與肝臟纖維化的相關(guān)性,cutoff值與APRI、FIB-4、FibroIndex多參數(shù)模型診斷效能的比較。結(jié)果以肝臟穿刺病理結(jié)果為金標(biāo)準(zhǔn),Spearman等級(jí)相關(guān)性分析提示LSM、APRI、FIB-4、FibroIndex多參數(shù)模型與肝纖維化相關(guān)(P0.05)。受試者工作特性曲線(ROC)分析,F1~F4期FibroTouch診斷肝纖維化分期的最佳cutoff值依次為6.6、7.7、10.5、15kPA;診斷有無顯著纖維化和肝硬化時(shí)的診斷價(jià)值優(yōu)于APRI、FIB-4、FibroIndex多參數(shù)模型。結(jié)論 FibroTouch在評(píng)估慢性肝病患者肝臟纖維化程度有較高的臨床應(yīng)用價(jià)值。
[Abstract]:Objective to evaluate the clinical value of instantaneous elastic ultrasound imaging (FibroTouch) and multi-parameter model in the diagnosis of hepatic fibrosis in patients with chronic liver disease and to evaluate the optimal diagnostic point (cutoff) value of FibroTouch in the diagnosis of hepatic fibrosis stage. Methods from January 2014 to October 2016, liver tissue biopsy was performed in 48 patients with chronic liver disease in our hospital. Serum routine examination, liver function and other serological indexes were collected, and liver elasticity (LSM) was measured with FibroTouch. To analyze the correlation between LSM and hepatic fibrosis and compare the diagnostic efficacy of APRII-FIB-4 FibroIndex multiparameter model. Results the correlation analysis of Spearman grade with the pathological results of liver puncture suggested that the multiparameter model of FIB-4 FibroIndex of LSMN APRIX was related to liver fibrosis (P0.05). The optimal cutoff value of F1F4 FibroTouch in diagnosing hepatic fibrosis stage was 6.6 / 7.710.5 / 15kPA, and the diagnostic value of FIB-4 FibroIndex was better than that of APRII-FIB-4 FibroIndex. Conclusion FibroTouch has a high clinical value in evaluating the degree of liver fibrosis in patients with chronic liver disease.
