【摘要】：目的探討雙氣囊小腸鏡檢查(DBE)對(duì)小腸出血患者的診斷時(shí)間選擇。方法系統(tǒng)回顧分析2010年1月-2014年10月該院消化科收治的可疑小腸出血患者121例臨床資料,按照檢查時(shí)間的不同分為A組(24 h內(nèi)行DBE),B組(24~48 h內(nèi)行DBE)和C組(48 h以后行DBE),收集整理3組患者的檢查結(jié)果,并進(jìn)行統(tǒng)計(jì)學(xué)分析及比較。結(jié)果 121例小腸出血患者全部接受DBE檢查,發(fā)現(xiàn)病變60例,病變總檢出率為49.59%。A組病變檢出率為67.50%(27/40),明顯高于C組的病變檢出率39.13%(18/46),差異有統(tǒng)計(jì)學(xué)意義(P=0.009);同時(shí)A組也高于B組的病變檢出率42.86%(15/35),差異有統(tǒng)計(jì)學(xué)意義(P=0.032)。而B(niǎo)組的病變檢出率與C組相比,差異無(wú)統(tǒng)計(jì)學(xué)意義(P=0.735)。24 h內(nèi)的行DBE檢查的平均檢查時(shí)間明顯短于24~48 h內(nèi)行DBE和48 h以后行DBE,差異有統(tǒng)計(jì)學(xué)意義;而24~48 h行DBE的平均檢查時(shí)間與48 h以后行DBE差異不大,差異無(wú)統(tǒng)計(jì)學(xué)意義。結(jié)論對(duì)于小腸出血患者,在24 h內(nèi)行DBE的檢查是安全、可行并且有效的,與24 h后DBE相比,檢查時(shí)間更短,病變檢出率更高。條件合適時(shí),對(duì)小腸出血患者可盡早行雙氣囊小腸鏡檢查。
[Abstract]:Objective to evaluate the diagnostic time of double balloon enteroscopy (DBE) in patients with small intestinal hemorrhage.Methods from January 2010 to October 2014, 121 patients with suspected small intestinal hemorrhage were analyzed retrospectively.According to the time of examination, the patients in group A were divided into two groups: group A received DBE within 24 h and group C received DBE within 48 h. The results of the three groups were collected and compared.Results all the 121 cases of small intestinal hemorrhage were examined by DBE and 60 cases were found to have pathological changes.The total detectable rate of lesions in group A was 67.50 / 40, which was significantly higher than that in group C (39.1318 / 46), the difference was statistically significant, and that in group A was higher than that in group B (42.86 / 35 / 35), the difference was statistically significant (P0.032).The detection rate of DBE in group B was significantly shorter than that in group C within 24 hours (P < 0. 735) and after 48 hours (P < 0. 735).However, there was no significant difference in the average examination time between 24 h and 48 h after DBE and DBE after 48 h, and there was no significant difference between the two groups.Conclusion DBE is safe, feasible and effective in patients with small intestinal hemorrhage within 24 hours. Compared with DBE after 24 hours, the examination time is shorter and the detection rate of lesions is higher.When conditions are appropriate, double balloon enteroscopy can be performed as soon as possible for patients with small intestinal hemorrhage.
