【摘要】：目的探討連續(xù)性血液凈化對重癥急性胰腺炎患者腎素-血管緊張素-醛固酮系統(tǒng)(RAAS)及炎癥因子的影響。方法收集該院ICU 2010年6月至2014年6月診治的重癥急性胰腺炎患者100例,按照隨機數字表法分為觀察組和對照組各50例,兩組均給予常規(guī)治療,觀察組在常規(guī)治療的基礎上給予連續(xù)性血液凈化治療,觀察兩組治療效果以及對RAAS及炎性指標的影響。結果經過3 d時間治療,觀察組急性生理和慢性健康(APACHE)Ⅱ評分、腹痛緩解時間、住院時間明顯低于對照組(P0.05),觀察組并發(fā)癥和死亡病例比例分別為14.0%、12.0%,明顯低于對照組的42.0%、36.0%(P0.05);RAAS指標血管緊張素(Ang)-Ⅰ、Ang-Ⅱ、腎素(E)、醛固酮(ALD)水平在兩組均為下降趨勢,各項指標經過方差分析P0.05。Ang-I、Ang-Ⅱ、E、ALD等指標治療前兩組差異均無統(tǒng)計學意義(P0.05),治療1、3 d觀察組明顯低于對照組(P0.05);炎癥指標腫瘤壞死因子(TNF)-α、白細胞介素(IL)-1β、IL-6、IL-8水平在兩組均為下降趨勢,各項指標經過方差分析P0.05。TNF-α、IL-1β、IL-6、IL-8等指標治療前兩組差異均無統(tǒng)計學意義(P0.05),在治療1、3 d測定TNF-α、IL-1β、IL-6、IL-8水平,觀察組的水平明顯低于對照組(P0.05)。結論連續(xù)性血液凈化對重癥急性胰腺炎患者治療效果明顯,可以明顯改善RAAS及炎性因子指標。
[Abstract]:Objective to investigate the effects of continuous blood purification on renin-angiotensin-aldosterone system (Ras) and inflammatory factors in patients with severe acute pancreatitis (SAP). According to the method of random digital table, the two groups were divided into two groups: the observation group (n = 50) and the control group (n = 50). Both groups were given routine treatment, and the observation group was given continuous blood purification therapy on the basis of routine treatment.
