結(jié)膜松弛癥患者淚膜穩(wěn)定性、杯狀細(xì)胞及黏蛋白5AC表達(dá)的變化

【摘要】:目的檢測和觀察結(jié)膜松弛癥患者淚膜穩(wěn)定性、杯狀細(xì)胞變化及黏蛋白5AC的表達(dá)情況,探討結(jié)膜松弛癥的發(fā)病機(jī)制。方法收集在我院就診的結(jié)膜松弛癥患者30例30眼)為結(jié)膜松弛癥組,另選取單純老年性白內(nèi)障手術(shù)患者15例(15眼)為正常對照組,對所有入選者進(jìn)行眼部癥狀評價(jià)[國際眼表面疾病指數(shù)(ocular surface disease index,OSDI)積分法]、淚膜破裂時(shí)間檢測、Schirmer I試驗(yàn)、淚液羊齒狀結(jié)晶試驗(yàn),分別對結(jié)膜松弛癥組和正常對照組的結(jié)膜組織標(biāo)本行HE染色、AB染色、黏蛋白5AC免疫組織化學(xué)染色觀察,行統(tǒng)計(jì)學(xué)分析。結(jié)果結(jié)膜松弛癥組OSDI評分為(37.80±8.94),高于正常對照組(11.40±4.08)(P0.01)。結(jié)膜松弛癥組淚膜破裂時(shí)間為(6.70±2.76)s,低于正常對照組(13.67±3.48)s(P0.01)。SchirmerⅠ試驗(yàn)結(jié)膜松弛癥組為(6.23±3.13)mm,明顯低于正常對照組(13.40±3.74)mm(P0.01)。結(jié)膜松弛癥組患者淚液中羊齒狀結(jié)晶較正常對照組明顯減少(χ~2=14.309,P=0.003)。光鏡下結(jié)膜松弛癥組較正常對照組球結(jié)膜厚度變薄,膠原纖維少,彈力纖維缺失,固有層間質(zhì)淤血水腫,杯狀細(xì)胞減少,黏蛋白5AC染色陽性細(xì)胞數(shù)低于正常對照組(χ~2=9.499,P=0.023)。結(jié)論結(jié)膜松弛癥患者淚膜功能受到影響,杯狀細(xì)胞減少,淚液中羊齒狀結(jié)晶減少,黏蛋白5AC含量減少,最終導(dǎo)致球結(jié)膜過度松弛,出現(xiàn)眼表淚液學(xué)異常。
[Abstract]:Objective to detect and observe the stability of tear film, the changes of goblet cells and the expression of mucin 5AC in patients with conjunctival relaxation, and to explore the pathogenesis of conjunctival relaxation. Methods 30 patients (30 eyes) with conjunctival relaxation in our hospital were collected as conjunctival relaxation group, and 15 patients (15 eyes) with simple age-related cataract surgery were selected as normal control group. All the selected patients were evaluated by international eye surface disease index (ocular surface disease index,OSDI) integral method, tear film rupture time was measured by, Schirmer I test, lacrimal goat tooth crystallization test. The conjunctival tissue specimens of conjunctival relaxation group and normal control group were observed by HE staining, AB staining and mucin 5AC immunohistochemical staining, and statistical analysis was performed.
