【摘要】：目的分析研究內耳結構異常患者人工耳蝸植入術的安全性及手術后的療效情況。方法選取2011.07-2015.06在我院住院接受人工耳蝸植入的17例內耳結構異常患者的臨床資料,其中語前聾14例,語后聾3例,針對內耳結構異常情況采用不同術式。用聽覺分級行為標準(Categorise of Auditory Performance,CAP)、言語可懂度分級標準(Speech Intelligibility Rating,SIR)進行術后效果評估。選取24例同期接受人工耳蝸植入的內耳正常的語前聾患者作為對照組,分析并比較兩組患者手術并發(fā)癥及術后療效。結果:17例患者手術順利,術后出現(xiàn)1例眩暈,所有患者術后均無腦脊液耳漏、面癱等并發(fā)癥。內耳結構異常語前聾患者術后CAP和SIR值分別為5.8±1.4、3.5±1.1;對照組CAP和SIR值分別為6.9±1.3、3.8±0.9,兩者差異無統(tǒng)計學意義(P0.05)。結論內耳結構異常非人工耳蝸植入術的絕對禁忌證,根據(jù)異常程度制定個性化的手術方案,可以減少并發(fā)癥的發(fā)生。內耳結構異常語前聾患者術后言語康復情況、對聲音的自發(fā)性覺察能力與耳蝸形態(tài)正�；颊咝g后無明顯差異。
[Abstract]:Objective to study the safety and efficacy of cochlear implantation in patients with abnormal inner ear structure. Methods the clinical data of 17 patients with abnormal inner ear structure who received cochlear implant in our hospital from June 7 to June 2011.07-2015.There were 14 cases of prelingual deafness and 3 cases of postverbal deafness. The postoperative effect was evaluated by auditory grading behavior Standard (Categorise of Auditory Performance,CAP) and speech intelligibility rating Standard (Speech Intelligibility Rating,SIR). Twenty-four patients with normal preverbal deafness received cochlear implants were selected as control group. The operative complications and postoperative outcomes were analyzed and compared between the two groups. Results: the operation was successful in 17 patients and 1 patient had vertigo after operation. No cerebrospinal fluid ear leakage and facial paralysis were found in all the patients. The values of CAP and SIR were 5.8 鹵1.4 鹵3.5 鹵1.1 in the patients with preverbal deafness and 6.9 鹵1.3 鹵3.8 鹵0.9 in the control group, respectively. There was no significant difference between the two groups (P0.05). Conclusion Non-cochlear implants with abnormal inner ear structure can reduce the incidence of complications by making individualized operative plan according to the degree of abnormal cochlea implantation. There was no significant difference in speech rehabilitation between the patients with abnormal inner ear structure and normal cochlea after operation.
