【摘要】：目的探討慢性鼻-鼻竇炎上頜竇口黏膜固有層病理學(xué)改變與鼻內(nèi)鏡手術(shù)預(yù)后之間的關(guān)系,通過有意義的病理學(xué)指標(biāo)以指導(dǎo)對(duì)該病的臨床診治。方法選取接受鼻內(nèi)鏡鼻竇手術(shù)的慢性鼻-鼻竇炎患者50例,術(shù)中取所選上頜竇口黏膜,進(jìn)行HE染色,觀察其固有層腺體面積與炎細(xì)胞數(shù)量2個(gè)病理學(xué)指標(biāo),術(shù)后6個(gè)月利用SNOT-20量表和Lund-Kennedy評(píng)分對(duì)所選患者進(jìn)行療效評(píng)估。結(jié)果慢性鼻-鼻竇炎患者的術(shù)后癥狀改善的好壞與黏膜固有層腺體面積直接相關(guān)(P0.05),而與固有層炎細(xì)胞數(shù)量無直接關(guān)聯(lián)(P0.05)。結(jié)論黏膜固有層腺體面積是反映鼻竇疾病嚴(yán)重程度的重要指標(biāo),能夠反映患者預(yù)后情況,對(duì)鼻內(nèi)鏡手術(shù)療效的預(yù)測(cè)具有重要意義。
[Abstract]:Objective to investigate the relationship between the pathological changes of the mucosal lamina propria of maxillary sinusitis and the prognosis of endoscopic sinus surgery in patients with chronic rhinosinusitis. Methods 50 patients with chronic rhinosinusitis underwent endoscopic sinus surgery were selected. The maxillary sinus mucosa was selected for HE staining. The area of the lamina propria and the number of inflammatory cells were observed. SNOT-20 scale and Lund Kennedy score were used to evaluate the curative effect of the selected patients 6 months after operation. Results the improvement of postoperative symptoms in patients with chronic rhinosinusitis was directly related to the area of glands in the lamina propria (P0.05), but not to the number of cells in laminitis propria (P0.05). Conclusion the area of the mucosal lamina propria is an important index to reflect the severity of sinus disease and can reflect the prognosis of the patients. It is of great significance to predict the outcome of endoscopic sinus surgery.
